Background Access time to emergency patients is a critical factor that affects the outcomes of life-or-death situations, especially in the cases of out-of-hospital cardiac arrests (OHCA). This study focused on developing a new model of emergency medical services (EMS) using a motorcycle-based ambulance (motorlance) with an automated external defibrillator (AED). There are currently no studies regarding access time for this vehicle. This study aimed at utilization of an AED in conjunction with motorlance and comparing the response time between a traditional ambulance and a motorlance. Methods This was a prospective study conducted in the EMS department of Srinagarind Hospital, located in Khon Kaen, Thailand, over a five-month period, from September 2021 to January 2022. Data were recorded employing a national standard of operations record form used for Thailand EMS departments nationwide. Results The 891 cases were divided into two groups which were motorlance and ambulance. The activation times for motorlance and ambulance were 0.44 minutes and 1.42 minutes, respectively (p < 0.001) and the response time in the motorlance group was 7.20 minutes compared with 9.25 minutes in the ambulance group. In OHCA, the motorlance with AED arrived at patients location and assisted to continue resuscitation at the hospital 88.9% of the time. Conclusion AED used in conjunction with motorcycle ambulances had shorter periods of both activation time and response time compared to ambulances. The use of AEDs clearly increases the number of continuous resuscitations in out-of-hospital cardiac arrest patients.

Introduction: Pre-hospital care covers assistance and immediate care to injured and seriously ill patients at the scene and during transfer to health facilities. Pre-hospital care demands skills and preparedness. Medical emergencies involve use of a range of transports other than ambulances in Nepal. The current pilot study aimed to explore the access to pre-hospital care and factors associated with it from both users and providersâ€™ perspective. Method: Cross sectional concurrent parallel mixed method study was done in emergency department (ED) of Patan Hospital, Nepal. Semi-structured questionnaire and interview guides were used. Thirty patients and visitors arriving at ED and seven key informants participated. Permission was taken from ED and written informed consent was taken. Descriptive and inferential statistics was done using R software and thematic analysis was done using EZR package. Result: Proportion of access to pre-hospital (PH) care was found to be 13.3% (4 out of 30). Concept of pre-hospital care itself is not popular among users. Limited PH care and service was found in the study site. Cost of transportation and distance between site and hospital were found to be associated with access. Further to it, barriers and facilitators were related with service delivery, workforce, financing, infrastructure, communication, coordination and information. Conclusion: Access to pre-hospital services was found only in 4 out 30 patients. Awareness among the usersâ€™ and integrated governance of ambulances is recommended for improvement of pre-hospital services. Factors influencing drivers' queue-jumping behavior at urban intersections: A covariance-based structural equation modeling analysis

<abstract> <p>Queue-jumping is widely acknowledged as one of the most vexing driving behaviors and a prevalent traffic violation at urban intersections in China, exerting detrimental effects on both traffic operational efficiency and safety. To investigate the motivational factors underlying drivers' queue-jumping behavior at urban intersections, a questionnaire was designed to collect data based on an extended theory of planned behavior (TPB). A total of 427 valid responses were received through an online self-reported questionnaire survey conducted in China. The Pearson's chi-square test was employed to examine potential demographic disparities in self-reported queue-jumping behavior among drivers at urban intersections. Covariance-based structural equation modeling (CB-SEM) with bootstrapping was utilized to elucidate the impact of various factors on drivers' engagement in queue-jumping behavior. The findings revealed significant gender and age differences regarding drivers' propensity for queue-jumping at urban intersections, with male and young drivers exhibiting higher inclination compared to female and older counterparts, respectively. Furthermore, the extended TPB effectively accounted for both behavioral intention and actual occurrence of queue-jumping among drivers at urban intersections. Behavioral intention (<italic>β</italic> = 0.391, <italic>p</italic> = 0.002) and perceived behavior control (<italic>β</italic> = 0.282, <italic>p</italic> = 0.002) emerged as influential determinants of queue-jumping. Among all influencing factors shaping drivers' behavioral intention toward engaging queue-jumping at urban intersections, attitude (<italic>β</italic> = 0.316, <italic>p</italic> = 0.005) proved to be the most significant factor followed by perceived risk (<italic>β</italic> = 0.230, <italic>p</italic> = 0.001), moral norms (<italic>β</italic> = 0.184, <italic>p</italic> = 0.002), subjective norms (<italic>β</italic> = 0.175, <italic>p</italic> = 0.002), and perceived behavior control (<italic>β</italic> = 0.122, <italic>p</italic> = 0.05). These results offer valuable insights for urban road traffic managers seeking effective strategies for public awareness campaigns as well as practical intervention measures aimed at curbing improper driving behavior of queue-jumping at urban intersections.</p> </abstract>
