The precise pathophysiological mechanisms of preeclampsia (PE) and preventative strategies remain unknown. Laboratory markers which can help in identifying PE patients from pregnant women and assessing the severity of PE during pregnancy are worthy to be explored. In this study, a retrospective case-control study was designed to assess whether the serum levels of albumin (ALB), total protein (TP), prealbumin (PA), alkaline phosphatase (ALP), lactic dehydrogenase (LDH), D-dimer, fibrinogen (Fbg), platelet (PLT) count, mean platelet volume (MPV), and platelet distribution width (PDW) can help in assessing PE and evaluate its severity. 256 pregnant women were enrolled and classified into 3 groups: mild preeclampsia (mPE, n = 85), severe preeclampsia (sPE, n = 78), and healthy normotensive controls (control, n = 93). Aim To explore whether neutrophil‐to‐lymphocyte ratio (NLR) and monocyte‐to‐lymphocyte ratio (MLR) could predict missed abortion (MA) at 7–13 week's gestation. Methods A total of 363 women with a diagnosis of MA and 232 women with normal pregnancy at 7–13 week's gestation, who visited our hospital from June 2012 to May 2018 were retrospectively analyzed. At 7 week's gestation, total and differential leukocyte counts, NLR and MLR were compared between women with MA (n = 69) and normal controls (n = 53). The receiver operating characteristic curve was used to select the optimal indicator and its cut‐off point. The role of the optimal indicator was further assessed at 8–13 week's gestation. Results The mean white blood cell counts, the mean neutrophil counts, the median monocyte counts, the mean NLR and the median MLR in women with MA were significantly lower than those in normal controls (P < 0.05, respectively).The neutrophil counts had the highest area under the curve (AUC) value of 0.772 (95% confidence interval 0.675–0.869) with a cut‐off value of 4.870 × 109/L, and the sensitivity was 72.46%, the specificity was 69.81%, positive predictive value was 75.76%, and negative predictive value was 66.07%. In addition, the neutrophil counts were also significantly lower in MA groups than those in normal controls at 8–13 week's gestation, and all had the highest AUC values. Conclusion Neutrophil counts may predict MA in the first trimester of pregnancy, which may provide a promising marker to diagnose missed abortion as early as 7 week's gestation.

Background/AimImmune tolerance in the fetal–maternal junction is maintained by a balance in the Th1/Th2 system. Th1‐type immunity is associated with pro‐inflammatory cytokines and immune checkpoint molecules (ICMs) such as B7‐H1, while Th2‐type immunity is characterized by anti‐inflammatory cytokines and ICMs such as B7‐1. Any imbalance in the Th1/Th2 immune system may lead to adverse pregnancy outcomes such as pre‐eclampsia (PE). Hitherto, the potential of serum B7‐1 and B7‐H1 proteins as early markers of PE has not been explored in the Ghanaian population.Materials and MethodsThis was a case–control study from May 2020 to April 2022 at the War Memorial and the Upper East Regional Hospitals. The study involved 291 women, including 180 (61.9%) with normotensive pregnancy and 111 (38.1%) with PE. Venous blood samples were collected and assayed for blood cell count, serum interleukins (ILs)‐4, ‐6, ‐12, ‐18, and TNF‐α as well as serum B7‐1 and B7‐H1 proteins.ResultsThe monocyte count (p = .007), the serum levels of IL‐18 (p = .035), TNF‐α (p = .001), and B7‐H1 (p = .006) were significantly higher in PE than in normotensive pregnancy. In addition, the monocyte count (p = .002), the serum levels of IL‐12 (p = .029), TNF‐α (p = .016), and B7‐1 (p = .009) levels were significantly higher in the third trimester than the second trimester PE. In predicting PE, the area under the curve of cytokines and ICMs ranged from 0.51 for IL‐6 to 0.62 for TNF‐α.ConclusionPE may be characterized by a dominant Th1‐type immunity with higher levels of pro‐inflammatory cytokines and B7‐H1 proteins, but these variables may not be suitable for predicting PE.
