This study aimed to evaluate the effect of anisodamine on regional cerebral blood flow (rCBF) and associateddizziness. 150 patients with dizziness induced by low rCBF were divided randomly into groups A (n = 60; anisodamine), P (n = 60; alprostadil), and C (n = 30; normal saline). rCBF and dizziness severity were evaluated. After treatment, rCBF values increased both in groups A and P. The subjective symptom of dizziness improved in 55 (91.7%) patients with the DHI score decreasing from 65.9 ± 5.4 to 23.1 ± 7.4 in group A, and the symptom improved in 37 (61.7%) patients with the DHI score decreasing from 66.8 ± 6.2 to 43.8 ± 8.6 in group P. The difference in DHI score and rCBF values in group A was more significant than that in group P. Anisodamine could increase rCBF and alleviate symptoms of dizziness more effectively than alprostadil.

Objective: This study was designed to assess the relationship between cases of vertigo and Hashimoto’s thyroiditis (HT) using vascular flow measured with vertebral Doppler ultrasonography (US). Materials and Methods: Patient cases that underwent vertebral Doppler US, thyroid hormone, and autoantibody tests between January 2020 and November 2021 were retrospectively reviewed. Hashimoto’s thyroiditis cases with associated vertigo were classified as the patient group, and other cases without vertigo and a known disease were the control group. A total of 184 patient cases were reviewed. Results: The HT patient group comprised 82 cases, and the control group comprised 102 cases. The median age was 43 years, and the interquartile range was 59 for all cases. The median total vertebral artery flow rate was 289.30 mL/min in the patient group and 278.35 mL/min in the control group, with no statistical difference ( P > .05). Conclusion: This study is the first to compare patients with HT and vertigo and measure vertebral vascular flow using Doppler US. This study demonstrates that vertigo in HT cases may not be due to vertebrobasilar insufficiency. The vertebral flow in the cohort was normal, and vertigo may have resulted from the autoimmune system.

The scope of the review is the problem of differential diagnosis between stroke and peripheral vestibulopathy in patients with acute vestibular vertigo. A vertebrobasilar stroke manifesting with the isolated vertigo has been previously recognized to be extremely rare, and the symptoms have been related to the involvement of peripheral parts of the vestibular analyzer. Recently there has been growing evidence that the isolated vertigo syndrome is commonly related to the central involvement of the vestibular analyzer. The author presents published clinical cases of acute cerebrovascular accident with a single symptom of acute vestibular vertigo. It can be also a symptom of a hemispheric stroke due to an injury of vestibular pathways connecting the vestibular nuclei with the parietal cortex. These observations extend the understanding of the common classic pathognomonic picture of central vestibular vertigo, which implies that its development is related exclusively to the brain matter lesion in vestibulobasilar stroke. Current clinical rating scales and tests (NIHSS, FAST) used for the diagnosis of an acute stroke, are frequently not sensitive to the vertebrobasilar stroke, and neuroimaging, including brain magnetic resonance imaging at DWI mode, may give false negative results. The most informative differential diagnostic method in acute vestibular syndrome is an otoneurological assessment including identification of nystagmus characteristics and head turn impulse test, for the assessment of vestibuloocular reflex and at bed tests (for example, tests included into the HINTS PLUS protocol). In this regard, it is important that neurology specialists in regional vascular centers and departments for acute cerebrovascular care should master the otoneurological assessment skills.
