Atopic dermatitis, the most common form of eczema, is a chronic, relapsing inflammatory skin condition that occurs with dry skin, persistent itching, and scaly lesions. This debilitating condition significantly compromises the patient’s quality of life due to the intractable itching and other associated factors such as disfigurement, sleeping disturbances, and social stigmatization from the visible lesions. The treatment mainstay of atopic dermatitis involves applying topical glucocorticosteroids and calcineurin inhibitors, combined with regular use of moisturizers. However, conventional treatments possess a certain degree of adverse effects, which raised concerns among the patients resulting in non-adherence to treatment. Hence, the modern use of moisturizers to improve barrier repair and function is of great value. One of the approaches includes incorporating bioactive ingredients with clinically proven therapeutic benefits into dermocosmetics emollient. The current evidence suggests that these dermocosmetics emollients aid in the improvement of the skin barrier and alleviate inflammation, pruritus and xerosis. We carried out a critical and comprehensive narrative review of the literature. Studies and trials focusing on moisturizers that include phytochemicals, natural moisturizing factors, essential fatty acids, endocannabinoids, and antioxidants were identified by searching electronic databases (PubMed and MEDLINE). We introduce the current knowledge on the roles of moisturizers in alleviating symptoms of atopic dermatitis. We then further summarize the science and rationale of the active ingredients in dermocosmetics and medical device emollients for treating atopic dermatitis. Finally, we highlight the limitations of the current evidence and future perspectives of cosmeceutical research on atopic dermatitis.

The experience of itch often poses a burden on patient quality of life and has the capacity to inflict significant suffering. Topical therapies are a mainstay of treatment for many cutaneous and systemic diseases and afford patients the opportunity to manage their conditions without many of the systemic side effects of non-topical therapies. We review a multitude of new topical medications targeting the skin, immune system, and neural receptors. The list includes Janus kinase inhibitors, tyrosine kinase inhibitors, phosphodiesterase inhibitors, transient receptor vanilloid inhibitors, topical cannabinoids, and topical acetaminophen. Many of the topical therapies reviewed show promising data in phase 2–3 clinical trials, but further research is needed to compare therapies head-to-head and test their efficacy on a broader range of conditions.

Background: Existing topical psoriasis treatments are partially effective or have long-term side effects for a proportion of people with psoriasis; therefore, effective and safe treatment options are required. Cannabidiol, a cannabinoid in Cannabis sativa, reverses the etiology of psoriasis through skin receptors according to in vitro research. Cannabidiol transdermal patches may be an effective treatment for psoriasis, although the efficacy and safety data are limited. Methods and analysis: This is a randomized double-blind controlled trial comparing cannabidiol (CBD) with minimal tetrahydrocannabinol (THC) patches with placebo patches (1:1 ratio) daily applied to comparable lesions of each patient with mild to moderate plaque-type psoriasis performed in a university hospital in Thailand (n=60). The primary outcome is the local psoriasis severity index (LPSI). The local severity index of psoriasis, the itch score using the visual analog scale, and adverse events will be evaluated on day 0, 30, 60, and 90 of the study. Furthermore, on days 0 and 90 of this study, biological samples will be taken for the evaluation of the skin, gut, and mouth microbial profile of 50% of randomly selected individuals. Conclusions: This study aims to investigate the efficacy and safety of cannabidiol transdermal patches in alleviating the symptoms of psoriasis. We will also examine personal impacts on the efficacy and safety of patches, such as the microbial profile. The results of this study may highlight a novel topical treatment option that reduces suffering in patients with psoriasis. Ethics and dissemination: This study was registered with the Thai Clinical Trials Registry (TCTR No. 20220518004). The protocol is being considered by the Institutional Review Board of the Faculty of Medicine, Chulalongkorn University. The results of this study will be faithfully presented through conferences or published articles.
