Assessing fidelity to evidence-based practices in usual care: The example of family therapy for adolescent behavior problems

Abstract: This study describes a multimethod evaluation of treatment fidelity to the family therapy (FT) approach demonstrated by front-line therapists in a community behavioral health clinic that utilized FT as its routine standard of care. Study cases (N = 50) were adolescents with conduct and/or substance use problems randomly assigned to routine family therapy (RFT) or to a treatment-as-usual clinic not aligned with the FT approach (TAU). Observational analyses showed that RFT therapists consistently achieved a leve… Show more

“…In a separate study (Hogue and Dauber 2013) we utilized benchmarking analyses to compare FT fidelity scores for sessions held at the RFT site to fidelity scores from an efficacy trial of an empirically supported treatment for ABPs, multidimensional family therapy (MDFT). We randomly selected 15 archived RFT sessions that were videotaped on site prior to the start of the current study.…”

“…In a separate study (AUTHOR, 2013b) we utilized observational benchmarking analyses to compare FT fidelity scores for sessions previously held at the UC-FT site to fidelity scores from an efficacy trial of MDFT. We randomly selected 15 archived UC-FT treatment sessions videotaped on site prior to the start of the current study by a previous cohort of therapists.…”

“…This process is referred to as testing the fidelity of the model or theory. As discussed by Hogue and Dauber (2013) fidelity consists of three related components: adherence, which is the quantity or extent to which interventions are delivered; competence, which is the quality or skill of delivery; and differentiation, which is the degree to which comparative intervention approaches differ from one another in practice based on a guiding theory and prescribed services and supports.…”
