Aspirin Plus Clopidogrel Versus Aspirin Alone After Coronary Artery Bypass Grafting

Abstract: Background-Clopidogrel inhibits intimal hyperplasia in animal studies and therefore may reduce saphenous vein graft (SVG) intimal hyperplasia after coronary artery bypass grafting. The Clopidogrel After Surgery for Coronary Artery DiseasE (CASCADE) study was undertaken to evaluate whether the addition of clopidogrel to aspirin inhibits SVG disease after coronary artery bypass grafting, as assessed at 1 year by intravascular ultrasound. Methods and Results-In this double-blind phase II trial, 113 patients under… Show more

“…However, this therapeutically approach needs randomized control trial with large study group to evaluate the benefit of such treatment. The benefit of such a treatment regimen has been investigated in CASCADE trial [26]. The authors investigated on the impact of dual antiplatelet therapy on venous graft disease in CABG patients.…”

“…To date, only persistent use of statin therapy and β blockers have been shown to reduce intimal hyperplasia in SVGs (9) while Edifoligide (8) and aspirin + clopidogrel (10), have both failed to do so, respectively, at 12-18 months after CABG. Mechanical external stents for SVGs have shown considerable promise in pre-clinical testing with reduction of proliferative intimal hyperplasia by reducing wall tension, improving lumen uniformity and creating a protective "neo-adventitia" layer rich with micro vasculature (11)(12)(13).…”

“…In the postoperative period, early aspirin therapy has been consistently reported to improve clinical outcomes for patients undergoing coronary artery bypass grafting (CABG), including better graft patency [3][4][5][6] and a reduced risk of death and ischemic complications [7]. However, whether aspirin should be used in the preoperative period remains controversial [8][9][10][11][12][13].…”
