Aspirin May Be a Suitable Prophylaxis for Patients with a History of Venous Thromboembolism Undergoing Total Joint Arthroplasty

Abstract: Background: In recent years, aspirin has become a popular agent for venous thromboembolism (VTE) prophylaxis following total joint arthroplasty (TJA). Yet patients with a history of VTE are often given more aggressive prophylactic agents because of their increased baseline risk. The purpose of this study was to assess whether aspirin is an effective prophylactic agent in patients with a history of VTE.Methods: This was a single-institution, retrospective cohort study. The electronic clinical records of 36,333 … Show more

“…One area where aspirin is not really used is in the prophylaxis of patients with a high risk of VTE due to prior history and therefore higher baseline risk, with surgeons shying away and preferring low molecular weight heparin in these situations. This paper from Philadelphia (Pennsylvania, USA) adds a lot to what is currently known by asking the question, ‘Could aspirin be used as prophylaxis in the majority of patients with a history of VTE?’ 7 The authors used their own single-institution registry to assemble a cohort study of 36,333 patients, all of whom underwent total joint replacements between 2008 and 2020. Of the whole cohort, 1,087 patients (3.0%) had a history of VTE.…”
