Asphericity of pretherapeutic tumour FDG uptake provides independent prognostic value in head-and-neck cancer

Abstract: Asphericity (ASP) characterises the spatial heterogeneity of FDG uptake in tumours. ASP is a promising prognostic parameter in head-and-neck cancer. ASP is useful for identification of high-risk patients with head-and-neck cancer.

“…Thirty-three patients, with LAHNC treated with RT-CT were included. Using the same cutoff of 20.4 found in (Apostolova et al, 2014), asphericity was correlated with PFS (HR 2.96, p = 0.015) and OS (HR 5.9, p = 0.001).…”

“…The authors validated their results on a dataset of 83 patients treated in the same institution after the original dataset (Tang et al, 2012). Based on (Apostolova et al, 2014), (Hofheinz et al, 2015) used a cutoff of TLG of 58.7 ml. They showed a correlation of TLG only with better DFS (HR 3.01, p = 0.048) but not with better OS (HR 2.02, p = 0.22).…”

“…Despite these interesting results, the possibility of adopting their model in the clinical practice is limited by the monocentric nature of their study and by the definition of the cut-off of MTV based on populationdependent characteristics (from the 25th to the 75th percentile). Based on the results of a previous study [32], Hofheinz et al [29] used a cut-off of 58.7 ml for TLG. They showed a correlation of TLG only with better disease-free survival (DFS; HR 3.01, p Z 0.048) but not with OS (HR 2.02, p Z 0.22).…”

“…Although many methods for evaluating the heterogeneity of intratumoral FDG uptake have been proposed [6,[9][10][11], cumulative SUV-volume histograms (CSH) and fractal analyses are considered to be simple methods for quantifying the intratumoral heterogeneity of FDG uptake [6,7]. CSH is obtained by plotting the percent area of a tumor with an SUV above a certain threshold against that threshold.…”
