Aspergillosis of the paranasal sinuses.

Abstract: The authors examined 105 proved cases of aspergillosis involving the paranasal sinuses or nasal fossa. Mycosis was always unilateral, and the maxillary sinus was infected in all cases. Early stages were manifested by an intraluminal soft-tissue mass representing the mass of mycelia. Fifty-nine patients (56%) demonstrated very dense intraluminal calcifications. Such dense concretions suggest the diagnosis of aspergillosis.

“…Currently, the presence of HAM is considered pathognomonic of ABPA [20]. The reason for the hyper attenuating mucus is probably similar to allergic fungal sinusitis and is currently attributed to the presence of calcium salts and metals (the ions of iron and manganese) [21] or desiccated mucus [22]. Our study showed mucoid impaction in the diameter of the bronchus divided by the external diameter of its accompanying artery) is > 1 [11].…”

“…The mucus plugs in ABPA are generally hypodense but can also be hyperdense; defined as a mucus plug visually denser than the paraspinal skeletal muscle [22] . The hyperattenuating character of mucus is ascribed to the presence of calcium salts and metals or desiccated mucus [46,47] . Recently, numerous reports have described the occurrence of HAM in ABPA [23,34,[48][49][50][51] , and currently the presence of HAM is considered a pathognomonic finding in ABPA [52] .…”

“…5,6 The ostiomeatal unit is subject to a large variety of anatomical variations in this region and are important as they may have pathological consequence or may be the source of difficulty/ complication during surgery. Stumberger et al proposed that stenosis of the ostiomeatal complex, from either the anatomical configuration or hypertrophied mucosa, can cause obstruction and stagnation of secretions that may become infected or perpetuate infection.…”

It is due to its negative influence on PNS ventilation and mucociliary clearance in the middle meatus region.…”
