Aspects of communication in Alzheimer's disease: clinical features and treatment options

Abstract: The capacity to treat or slow the progression of communication deficits in AD would prolong patient independence, and have a profound impact on the quality of life of patients and caregivers. The use of pharmacological (anti-AD therapies) and non-pharmacological (cognitive-linguistic stimulation) treatments may be useful management methods and warrant further investigation.

“…As a matter of fact, decline in language has been shown to correlate with noncognitive items, such as personal care, hobbies, occupations, and behavior. 53 Overall, despite the significant results in improving different aspects of PWA, our study is not without limitations. Although the improvement in cognitive and behavioral areas was preserved in the tMST group at 22 weeks, the length of follow-up was relatively short.…”

“…52 We consider this result clinically significant, because it can allow a reduction of the use of antipsychotic drugs, whose harmful effects on PWA is now well known. As for the clinical value of results in the cognitive domain, we believe that obtaining even a moderate improvement of language and memory is a meaningful result: memory impairment is the principal symptom of PWA and is a source of depression for them and of burden for caregivers (eg, due to repetitive questioning) 53 ; impairment of language, on the other hand, favors social isolation and has particular negative sequelae for the interaction with family members, increasing their burden of care. In the progression from moderate to severe stages in AD, worsening language abilities, or aphasia, has been suggested to have more clinical relevance than other domains.…”

“…Language and communication disturbances suffered by AD patients include [276] word recall and word-finding difficulties Pitch, standard deviation of pitch, max and min pitch, intensity, standard deviation of intensity, max and min intensity, period mean, period standard deviation, and root mean square amplitude, shimmer, local jitter, noise-to-harmonics ratio, harmonics-to-noise ratio and autocorrelation, fraction of locally unvoiced frames, degree of voice breaks HFD Max, min, variance, SD [277][278][279][280], repetitions during speech [277][278][279]281], loss of both reading and writing skills [280], problems to follow a conversation due to deterioration in concentration and comprehension skills [279,280] and decline in non-verbal communication skills [282]. These problems are present from the very early stages of the disease, and progress and worsen at the same time that the cognitive decline [276,283].…”

“…These problems are present from the very early stages of the disease, and progress and worsen at the same time that the cognitive decline [276,283]. Due to the ease with which voice recordings can be obtained, in the recent years, speech features have been used in works related to automatic MCI and AD diagnosis systems [27,40,[284][285][286][287][288].…”

“…Although studies described interventions with the aim of improving students' attitude, knowledge and level of comfort, it is perhaps surprising that none specifically addressed the need to improve (Woodward, 2013), indicating that advanced communication skills are required to communicate well and provide care effectively. Therefore this is an area needing development and evaluation in pre-registration education to ensure the future healthcare workforce is equipped with the necessary skills.…”
