Objective: To compare the epidemiological, the clinical and the paraclinical profile of osteoarticular infections in HIV-positive patients and in HIV-negative patients. Patients and Methods: patients with Osteoarticular infections recognized on the basis of radioclinical and epidemiological arguments were included in this study. The comparison that was based on the existence or was not of HIV infection focused on various clinical and biological components. Of these 15,800 rheumatic patients admitted in 18 years, 461 have suffered from Osteoarticular infection (2.9%). The 461 patient objects of this study were divided into 235 cases of infectious arthritis including 38 cases of co-infection with HIV and 225 cases of infectious spondylodiscitis including 21 cases of HIV coinfection. Results: There were 246 women (53.4%) and 215 men (46.4%) with a sex ratio of 1.1. The mean age was 37.7 ± 13.6 years. The data comparison of HIV-negative patients and HIV-positive patients had not shown statistically significant difference in the infectious spondylodiscitis group and in the group of infectious arthritis patients. Only fever was statistically significant in both groups and weight loss in the group of infectious arthritis (p < 0.001). Conclusion: HIV seems not to particularly influence the semiological profile of Osteoarticular infections in Lomé.

Objective: To research distribution of etiologies modification of arthritis in Congo-Brazzaville, twenty years after the first reports. Methods: A cross sectional study has been achieved. Medical files of patients admitted for arthritis between 2000 and 2014, in Rheumatology department of Brazzaville university teaching hospital have been included. Among 416 patients listed as cases of arthritis, 201 answered to the inclusion criterias have been kept for analysis. The etiological diagnoses were based on criterias of classification and/or diagnosis used in Rheumatology. Results: 201 patients, 110 men (54.72%) and 91 women (45.28%) were included. The sex-ratio was 1.2, and average age was 45.5 years old (extremes: 8 -86 years). Among them, 72 patients had microcrystal arthritis. Septic arthritis and those associated with HIV constituted the second etiological group of 60 patients and respectively, 32 were bacterial and 28 HIV associated arthritis. 58 remaining patients had a chronic inflammatory arthritis. Etiology distribution showed that gout was the most frequent (33.83%), followed by septic arthritis (15.92%), HIV associated arthritis (13.93%) and rheumatoid arthritis (11.94%). In 11 patients (5.5%), etiology was unknown. Conclusion: Three decades after the first publications in Brazzaville, the etiologies of arthritis remain dominated in order of frequency by gout, septic arthritis and HIV associated arthritis and rheumatoid arthritis. The frequency of indeterminate arthritis decreased significantly. Spondy-loarthropathy and autoimmune diseases are more common diagnosis.

Objectif : faire une revue des données de la littérature sur les aspects épidémiologiques, cliniques, paracliniques et thérapeutiques de la polyarthrite rhumatoïde au Niger. Méthodes : Cette revue des données de la littérature sur la polyarthrite rhumatoïde au Niger a été réalisée en utilisant les bases de données MEDLINE (via PUBMED) et de Google Scholar. Les critères d’inclusion comprenaient des études publiées chez des adultes de plus de 18 ans ayant reçu un diagnostic de polyarthrite rhumatoïde. L’extraction des données a été réalisée grâce à un formulaire préétabli. Résultats : La recherche dans la littérature a identifié 5 articles. Finalement, quatre articles ont été inclus pour la revue. La fréquence de la polyarthrite rhumatoïde a été rapportée dans trois études, celle-ci variait de 1,24% à 3,7%. Le genre féminin représentait 79 (79,79%) des patients. L’âge moyen globale était de 43,41 ans. Le délai moyen avant le diagnostic était de 53,2 mois. La polyarthrite rhumatoïde a été diagnostiqué en utilisant les critères ACR 1987 et les critères ACR/EULAR 2010. Tous les patients avaient consulté pour une polyarthrite. Le coup de vent cubital était retrouvé chez 27 (27,27%) patients. Les nodules rhumatoïdes ont été retrouvé chez 12 (12,12%) patients. Les facteurs rhumatoïdes étaient positifs chez 56 (56,56%) patients. Les anticorps anti peptides cycliques citrullinés étaient positif chez 7 (7,07%) patients. Le méthotrexate était le traitement de fond chez 67 (67,67%) patients et l’hydroxychloroquine chez 43 (43,43%) patients. Conclusion : Le diagnostic de la polyarthrite est tardif au Niger. La création d’un registre national des patients ayant une polyarthrite rhumatoïde permettra un meilleur suivi des patients. Ulnar gale was found in 27 (27.27%) patients. Rheumatoid nodules were found in 12 (12.12%) patients. Rheumatoid factors were positive in 56 (56.56%) patients. Anti-cyclic citrullinated peptide antibodies were positive in 7 (7.07%) patients. Methotrexate was the background treatment in 67 (67.67%) patients and hydroxychloroquine in 43 (43.43%) patients. Conclusion: Polyarthritis is diagnosed late in Niger. The creation of a national registry of rheumatoid arthritis patients will allow better follow-up of patients.
