Background: Heart Failure (HF) is a highly dysfunctional disease, with impact on the biopsychosocial dimensions of the patients. The worldwide progressive growth of HF incidence, caused by longer life expectancy, results in higher HF rehospitalization rates. HF patients’ rehospitalization is mostly a result of avoidable causes, including limitations in knowledge regarding HF self-care and impaired therapeutic adherence; therefore, the intervention of nurses is essential. Goals: To identify nursing interventions to address alterations in Basic Human Needs (BHN) and to promote relief of symptoms for HF patients as they transition from hospital to home, including follow-up care. Method: A systematic review in CINAHL and MEDLINE databases to answer the study question: In patients with HF, what nursing interventions help to improve BHN, promote relief of symptoms and aid the transition from hospital to home? Results: Nine studies were included. Nursing interventions focused on relief of symptoms during hospitalization to promote well-being and the individual’s health status. Preparing the patient’s transition from hospital to home earlier in their admission allows for the identification and adequate response to patient needs. Language adaptation during health education and a periodic follow-up were effective measures to promote independence in HF self-care and to reduce mortality and rehospitalization rates. Even though these interventions show positive results, they are not frequently used in clinical practice given a lack of specialized education about HF among healthcare professionals and empowerment culture, as well as an incapacity for adequate patient follow-up. Final Considerations: The nurse is responsible for helping the patient to develop skills to manage their symptoms (e.g., recognize relevant symptoms) and the therapeutic regimen; if successfully achieved, this promotes empowerment, thus reducing readmissions and promoting well-being.

As doenças cardiovasculares são as principais causa de óbitos e internações hospitalares no Brasil, vitimando cerca de 300.000 por ano, sendo assim, têm forte impacto na qualidade de vida e influência no aumento da permanência hospitalar do indivíduo, além de onerar os sistemas públicos e privados de saúde e seguridade social. Diante do exposto surgiu a questão norteadora: quais as principais causas de morte dos pacientes hospitalizados com doenças cardiovasculares? Para responder a tal questionamento, foi estabelecido como objetivo: identificar através da literatura, as principais causas de morte dos pacientes hospitalizados com doenças cardiovasculares. Trata-se de uma revisão integrativa da literatura, com abordagem qualitativa, realizada no período de março a maio de 2021. Realizou-se o cruzamento "Enfermagem" AND "Cardiologia" AND "Hospitalização", que após a aplicação dos filtros e análise completa, restaram 6 estudos para comporem a amostra da revisão. De janeiro a maio de 2021, 143.420 pessoas evoluíram para óbito no Brasil em decorrência de causas cardiovasculares, segundo o cardiômetro desenvolvido pela Associação Brasileira de Cardiologia. O estudo evidenciou a importância de uma análise epidemiológica quanto ao número de óbitos ocorridos em unidades hospitalares, destacando-se como objetivo a análise das ações de planejamento e organização de políticas e protocolos que ajudem os profissionais de saúde a abordarem os pacientes internados nesse setor e compreender as principais etiologias, disfunções e evolução quanto à idade, gênero e possíveis complicações.

AimThis research quantitatively explored the prevalence of NANDA‐I nursing diagnoses related to the care of patients experiencing heart failure.DesignA systematic review and meta‐analysis were conducted with the systematic review protocol registered in PROSPERO (registration number: CRD42022382565).MethodsSystematic searches were performed between March and April 2022, including peer review for selection, quality assessment, data extraction, and analysis of all included studies. A subsequent meta‐analysis was performed, focusing on the proportion of nursing diagnoses in patients with heart failure. The logistic random effects model with maximum likelihood estimation assessed the combined proportion, and heterogeneity between studies was evaluated using the I2 statistic. The goodness‐of‐fit of the meta‐analysis results was assessed using the leave‐one‐out method and by evaluating publication bias through contour‐enhanced funnel plots.Data sourcesPubMed, SCOPUS, CINAHL, WOS, and Embase were used.ResultsOf the 11 studies that met the eligibility criteria, 44 nursing diagnoses were identified as most frequently occurring in patients experiencing heart failure, and only 16 diagnoses appeared in more than one publication. The combined mean proportion was 35.73% (95% CI = [26.67%; 48.56%]), indicating the presence of heterogeneity based on the I2 value. However, no publication bias was observed.ConclusionThe results of the meta‐analysis suggest priority diagnoses in individuals with heart failure, such as deficient knowledge (00126). Additionally, secondary diagnoses, such as activity intolerance (00092), excess fluid volume (00026), and ineffective breathing pattern (00032), were identified as responses to decreased cardiac output (00029). Less prevalent nursing diagnoses were associated with deterioration of health status and the need for hospitalization.ObjetivoInvestigar cuantitativamente la prevalencia de diagnósticos de enfermería NANDA‐I asociados con la atención de pacientes con insuficiencia cardíaca.DiseñoRevisión sistemática y metanálisis. Además, se han identificado otros diagnósticos de enfermería como diagnósticos secundarios: Intolerancia a la actividad (00092), Exceso de volumen de líquidos (00026) y Patrón respiratorio ineficaz (00032), que sería la respuesta a Disminución del gasto cardíaco (00029). Además de diagnósticos de enfermeríaa menos prevalentes relacionados con el deterioro del estado de salud y la necesidad de hospitalización.
