ASCENDE-RT: An Analysis of Treatment-Related Morbidity for a Randomized Trial Comparing a Low-Dose-Rate Brachytherapy Boost with a Dose-Escalated External Beam Boost for High- and Intermediate-Risk Prostate Cancer

Abstract: The incidence of acute and late GU morbidity was higher after LDR-PB boost, and there was a nonsignificant trend for worse GI morbidity. No differences in the frequency of erectile dysfunction were observed.

“…It is interesting that there was only little correlation between urethral stricture and dose to the prostate in the ASCENDE-RT [12]. Similarly, in our meta-regression analysis, the BED did not show an association with urethral stricture development.…”

“…This study also found the highest incidence of urinary adverse events among RTs with EBRT + BT [11]. Furthermore, despite having an improved biochemical recurrence-free survival (b-RFS) by combining BT with EBRT [12], several studies also demonstrate an increased risk of developing urethral strictures or urinary toxicity compared to other treatment modalities [10][11][12][13][14]. Interestingly in our analysis, 23 studies used HDR-BT, and 18 out of these studies used HDR-BT as a boost with EBRT.…”

“…The toxicity of prostate UHRT relative to the combination of EBRT plus brachytherapy or other forms of moderate hypofractionated or conventional fractionated EBRT has been the subject of debate [5,36,37]. Figure 5 shows that late grade 2-5 toxicities in the present study were similar to those from centres using five-fraction UHRT and compare favourably with other radiation therapies.…”

“…To set in context the outcomes of the present trial, we gathered the data on BDFS rates and grade 3+ toxicity of several trials [5,7,8,[25][26][27][28][29][30] with a minimum 5-year follow-up ( Fig. 5).…”
