According to the studies carried out in advance, the medical area has analysed the patient's condition visually by applying a common methodology to all patients based on their knowledge of the disease, making a mistaken assessment due to the complexity of the disease, which varies according to the length of time the patient has been suffering from it. The problem posed by this disease rheumatoid arthritis is the bad evaluation by means of radiographs of the hands, relying on the criteria of the doctor when treating the disease with his experience, making serious mistakes when the quality of the image is not correct when interpreting a first stage disease with an advanced one, therefore, it is important to evaluate rheumatoid arthritis by means of an image treatment in a safe way for a better diagnosis of the patient. The aim of this research is to develop a hand joint position analysis system for the care of patients with rheumatoid arthritis to help doctors detect tuberculosis earlier and more accurately and to avoid prolonged infections that could be fatal for patients. The methodology used for this research is based on taking a computerized X-ray of both hands and applying image processing using a computer, the image processing is carried out using MATLAB by applying the various processing tools to detect these conditions. According to the tests performed, it was observed that the system detects the state of the patient with rheumatoid arthritis with an efficiency of 96.14% in its operation, standing out satisfactorily with respect to other systems for its high efficiency of image analysis of patients with the MATLAB tool. It concludes that this system can be used in different circumstances of the patient's condition, from the initial stage of arthritis to an advanced stage of the patient's disease.

Introducción La artritis reumatoide es una enfermedad inflamatoria crónica que presenta un gran impacto en el paciente y en los sistemas sociosanitarios. Los objetivos terapéuticos se basan en alcanzar la remisión o mantener la actividad más baja posible. Objetivo Determinar la actividad según DAS28 (Disease Activity Score con un conteo de 28 articulaciones) y los eventos adversos en artritis reumatoide durante el uso de metotrexato con leflunomida en el servicio de Reumatología del Complejo Hospitalario Dr. Arnulfo Arias Madrid, Panamá. Enero 2010- diciembre 2020. Metodología Se realizó un estudio observacional, descriptivo, retrospectivo y transversal, para lo cual se revisaron 278 expedientes clínicos electrónicos de pacientes con artritis reumatoide que han recibido terapia combinada con metotrexato más leflunomida en el servicio de Reumatología. Resultados El grupo etario con mayor número de casos fue 60-69 años, con predominio del género femenino. Según las puntuaciones de DAS28: 35.85% logró la remisión, 35.22% obtuvo baja actividad, 24.53% mantuvo moderada actividad y 4.40% persistió con alta actividad. Hubo 71.06% de eficacia. Dentro de los eventos adversos, se registraron: mucositis (0.63%), síntomas gastrointestinales (7.55%), citopenias hematológicas (13.21%), elevación de transaminasas (28.93%), alopecia (4.40%) e infecciones (6.29%). La mayoría de los acontecimientos adversos fueron leves y no ameritaron cambios en la terapia o resolvieron con la reducción de dosis de uno a ambos fármacos. Conclusión La terapia combinada con metotrexato más leflunomida ha demostrado ser una estrategia terapéutica costo efectiva en nuestro país, manteniendo un buen perfil de seguridad, sin eventos adversos graves que pudiesen comprometer la vida de los pacientes.

Rheumatoid arthritis (RA) is one of the most common autoimmune disorders affecting 0.5–1% of the population worldwide. As a disease of multifactorial etiology, its constant study has made it possible to unravel the pathophysiological processes that cause the illness. However, efficient and validated disease models are necessary to continue the search for new disease-modulating drugs. Technologies, such as 3D cell culture and organ-on-a-chip, have contributed to accelerating the prospecting of new therapeutic molecules and even helping to elucidate hitherto unknown aspects of the pathogenesis of multiple diseases. These technologies, where medicine and biotechnology converge, can be applied to understand RA. This review discusses the critical elements of RA pathophysiology and current treatment strategies. Next, we discuss 3D cell culture and apply these methodologies for rheumatological diseases and selected models for RA. Finally, we summarize the application of 3D cell culture for RA treatment.
