Arthroscopic Resection of Fat Pad Lesions and Infrapatellar Contractures

Abstract: The infrapatellar fat pad (IFP), also known as Hoffa's fat pad, may be a common site of pain in the knee because of its susceptibility to injury and its vast innervation and vascular supply. Patients who have trauma to the IFP may undergo a process of hemorrhage, inflammation, and fibrosis that may become painful. Patients with Hoffa's disease in whom conservative treatment with medications, physical therapy, and injections has failed may receive significant pain relief and benefit from undergoing arthroscopic… Show more

“…IPFP is routinely removed as surgical waste during knee arthroplasty, and it represents an accessible source of ASCs. In 1904, Hoffa described the surgical removal of IPFP, and after that Kumar mentioned an arthroscopic technique to ablate IPFP in 2007 17) . In brief, for the isolation of the IPFP during knee arthroplasty in our clinical setting, the tourniquet was inflated and then limited medial parapatellar skin incision (ca.…”

“…In order to put forward the IFP as a promising cell source for cartilage regeneration, it is important to understand its anatomical characteristics, as well as its developmental origin. As an adipose tissue within the joint, the IFP can be easily harvested arthroscopically or during open knee surgery [ 38 ]. The IFP is an intracapsular structure in the anterior knee compartment, composed of approximately 20 cm 3 of adipose tissue, or slightly larger in patellofemoral OA joints [ 39 41 ].…”

“…The infrapatellar fat pad (IPFP) is located in the knee joint [ 25 , 26 ]. IPFP can be easily resected during knee surgery either open or arthroscopically [ 27 ]. The isolation of MSCs has previously been reported from this source [ 28 ].…”
