Arthroscopic gel-type autologous chondrocyte implantation presents histologic evidence of regenerating hyaline-like cartilage in the knee with articular cartilage defect

Abstract: Purpose To investigate the clinical, radiological, and histological results of arthroscopic gel-type autologous chondrocyte implantation (GACI) in treating chondral defects of the knee. Methods This study prospectively examined five males and five females with a mean age of 40.3 ± 10.3 years who underwent arthroscopic GACI between March 2012 and February 2013. The gel comprised a mixture of 1 ml of fibrinogen plus 0.1-0.2 ml of thrombin. The mean size of chondral defect was 2.9 ± 1.2 cm 2 (range 1.2-5.4 cm 2 )… Show more

“…Table 4 includes studies available on PubMed of thirdgeneration ACI that included clinical and radiological results after 2 years. Results for the following scaffolds were available: the bilayer MACI technique, 6,32 the Novocart 3D, 25,39 the type I/III collagen membrane MACI (ACI-Maix), 11,20 the arthroscopic gel type ACI technique, 37 the cell-seeded ACI (which was compared with the secondgeneration collagen ACI), 15 the collagen-based CaReS scaffold, 36 the arthroscopically implanted chondrosphere, 33 a type I/III collagen membrane, 29 the 2-component gelpolymer scaffold BioSeed-C, 28 and the Hyalograft C. 17,36,38 They are, in general, good to excellent results, with some exceptions in the KOOS subscores, but the scores are comparable or similar among the different scaffolds. The MOCART score shows values between 70 and 85, all graded as "good," except in the study by Siebold et al 33 (score of 60), who used spheres instead of a matrix.…”

“…Several reports employed another technique for MACI, similar to the one we used. In a prospective study of ten patients undergoing arthroscopic gel-type autologous chondrocyte implantation (GACI), satisfactory clinical, radiological, and histological outcomes were reported, confirming the sufficient regeneration of hyaline-like cartilage, in parallel to the improved symptoms (Yoon et al, 2020). Since the GACI was infused in a liquid form, it was applied to the lesions regardless of their geometry, and the implant could then spread over the lesion site.…”

“…Several reports involved another technique for MACI just as in the current study. Yoon et al [19], in a prospective study, enrolling ten patients undergoing arthroscopic gel-type autologous chondrocyte implantation (GACI), manifested that GACI produced satisfactory clinical, radiologic, and histologic evaluation, which con rmed su cient regeneration of hyaline-like cartilage that correlated improved symptoms. Since the GACI was infused in liquid form, it was applied to the dependent position of the lesion regardless of its geometry, and the implant could then spread by itself over the lesion site.…”
