Arthroscopic excision of the ganglion in the posterior septum by posterior trans-septal portal: report of two cases

Abstract: We report two cases of ganglion cysts in the posterior septum of the knee joint, one as parameniscal cyst from the posterior horn of the lateral meniscus extending to posterior septum, and the other as a cyst located in the posterior septum adjacent to the posterior cruciate ligament, which were both arthroscopically excised expediently by posterior trans-septal portal. The posterior compartment of the knee is not readily accessible by ordinary arthroscopic portals, and therefore has been considered as 'blind … Show more

“…To date, articles on arthroscopic posterior trans-septal approaches for PCL reconstruction, suture of posterior horn of the medial meniscus or posterior capsules and excision of a ganglion cyst derived from the septum have been reported [8-12]. We have also used this posterior trans-septal approach to perform synovectomy of the posterior capsule, resection of the posterior horn of the medial meniscus, resection of PCL ganglion, and resection of free bodies in the posterior cavity.…”

“…17,18 The transseptal portal provides an excellent visualization of the posterior compartment and can be used in many troublesome situations. 15,17,[19][20][21][22][23] However, it is technically demanding and requires special care to avoid posterior neurovascular injuries. Therefore, finding the most suitable and safe position in different kinds of surgeries is important.…”

“…Various applications of the posterior transseptal portal have been proposed including total arthroscopic synovectomy of the posterior compartment, removal of loose bodies or tumors behind the PCL, suture fi xation of an avulsion of the PCL from the tibia, reconstruction of the PCL, and repair of the posterior horn or root of the medial meniscus. Th e popliteal artery, the most anterior structure of the popliteal neurovascular bundle, is clearly at risk during establishment of the trans-septal portal 15,16,[32][33][34][35] . To date, no cases of popliteal artery injury during creation of the trans-septal portal have been reported, to our knowledge.…”
