BackgroundSurgical treatment of temporomandibular joint (TMJ) disc displacement (DD) has been established in different forms since over a century. Ther is a consensus to perform minimal invasive interventions as first‐line surgical treatment since there are no evidence on best surgical practice yet.ObjectiveThe aim was to perform a complex systematic review (SR) on the topic—is there evidence for surgical treatment of TMJ DD?MethodsThe PICO was defined as DD patients (population), treated with different surgical interventions including arthrocentesis (intervention), compared with other or no treatment (control) regarding the outcome variables mandibular function, mouth opening capacity, TMJ pain, etcetera (outcome). For identification of prospective controlled trials and SRs, a search strategy was developed for application in three databases.ResultsThe search yielded 4931 studies of which 56 fulfilled the stipulated PICO. Studies with low or moderate risk of bias were possible to include in meta‐analyses. There were evidence suggesting arthrocentesis being more effective compared to conservative management (maximum interincisal opening (MIO): p < .0001, I2 = 22%; TMJ pain: p = .0003, I2 = 84%) and arthrocentesis being slightly more effective than arthrocentesis with an adjunctive hyaluronic acid injection (MIO: p = .04, I2 = 0%; TMJ pain: p = .28, I2 = 0%). Other treatment comparisons showed nonsignificant differences. The performed meta‐analyses only included 2–4 studies each, which might indicate a low grade of evidence.ConclusionAlthough arthrocentesis performed better than conservative management the findings should be interpreted cautiously, and non‐invasive management considered as primary measure. Still, several knowledge gaps concerning surgical methods of choice remains.

Introduction: Tempromandibular joint disorder is common disorder that has multifactorial etiology presented with cardinal clinical presentation of pain, joint sound and limitation of mouth opening. The most common causes of this disorder are muscular which are commonly referred as a myofascial pain and dysfunction other cause include internal derangement of TMJ structures. Objectives: This study was established to determine the effect of bite raising appliance in the treatment of myofacial pain and internal derangement of temporo-mandibular joint. Material & methods: This is a prospective study for 100 cases of temporo-mandibular joint disorder (TMD) patients were selected from department of maxillofacial surgery in Azadi Teaching Hospital, Kirkuk, Iraq and from the private clinic in the period of Jan-2012 to Dec. 2020.Results & conclusion: After 3 months of clinical follow-up the results revealed that bite raising appliance has a good outcomes in relieving pain, clicking and increase mouth opening in patient with myofacial pain and internal derangement of TMJ.

Background: Internal derangement (ID) is the most common cause for TMDs. Various invasive, minimally invasive, and non-invasive treatment modalities have been attempted. Arthrocentesis as a minimally invasive maneuver either if followed with various intra-articular medications or not has been attempted to treat ID. Hyaluronic acid (HA) has been used as an intraarticular medication due to its lubricant effect that improves joint movement and mechanically reduces friction pain. However, the i-PRF had both anti-inflammatory action to reduce the level of the cytokines; aided by the regenerative capacity to the torn and degenerated tissues of the joint including the synovial tissue. Purpose: The current study aimed at comparing both factors. Materials and Methods: Forty TMJs from thirty patients were randomly divided into two equal groups. In group I, arthrocentesis was followed by intra-articular injection of AH; however, in group II, arthrocentesis was followed i-PRF injection. Results: In general, group II patients recorded better results with respect to clicking, pain, and movement limitation. Superior results of i-PRF might be the result of the regenerative capability that not only reduced the existing inflammatory mediators; but also replaced the degenerated cytokines-releasing tissues by healthy synovial fluid-releasing tissues provides endogenous sustained source of HA. Conclusion: Although both treatments are acceptable, i-PRF was a rather recommended treatment.
