Reporting Outcomes and Outcome Measures in Digital Replantation: A Systematic Review

Purpose There is a lack of consensus on what the critical outcomes in replantation are and how best to measure them. This review aims to identify all reported outcomes and respective outcome measures used in digital replantation. Materials and Methods Randomized controlled trials, cohort studies, and single-arm observational studies of adults undergoing replantation with at least one well-described outcome or outcome measure were identified. Primary outcomes were classified into six domains, and outcome measures were classified into eight domains. The clinimetric properties were identified and reported. A total of 56 observational studies met the inclusion criteria. Results In total, 29 continuous and 29 categorical outcomes were identified, and 87 scales and instruments were identified. The most frequently used outcomes were survival of replanted digit, sensation, and time in hospital. Outcomes and measures were most variable in domains of viability, quality of life, and motor function. Only eight measures used across these domains were validated and proven reliable. Conclusion Lack of consensus creates an obstacle to reporting, understanding, and comparing the effectiveness of various replantation strategies.

Настоящий обзор основан на анализе отечественной и зарубежной литературы о хирургическом лечении повреждений дистальных фаланг пальцев кисти. Приведено описание имеющихся на настоящий момент клас-сификаций повреждений кончиков пальцев, возможных ошибок, допускаемых в ходе лечения на разных уров-нях оказания медицинской помощи. Особое внимание уделено современным методикам восстановления целостности кончиков пальцев кисти, оценке результатов лечения, наиболее часто встречаемым осложнениям и некоторым методам их устранения.Ключевые слова: дистальная фаланга, ноготь, травма кончика пальца, лоскуты, реплантация.This review is based on an analysis of literature on the surgical treatment of injuries of the distal phalanges of the fingers. The description of currently available several classifications of fingertip injuries and possible errors made during in the treatment at different levels of care are provided. Particular attention is paid to modern methods of restoring the integrity of the fingertips, assessing the treatment outcomes, the most common complications, and some methods for their elimination.Key words: distal phalanx, nail, fingertip injury, flaps, replantation.УДК 617.578-001-089.15 doi 10.17223/1814147/56/11 ВВЕДЕНИЕ Травмы дистальных фаланг пальцев (ДФП) кисти встречаются довольно часто во всем мире [7,33,40, 125]. D. Ootes и соавт. указывали, что самым частым типом травмы в США, заставляю-щим людей обратиться за неотложной медицин-ской помощью, являлась рваная рана пальца (221 случай на 100 тыс. населения ежегодно); перело-мы пальцев -на третьем месте (68 случаев на 100 тыс. населения ежегодно) [116]. С этими дан-ными коррелируют результаты исследований, проведенных в Великобритании, где частота пере-ломов пальцев составляет 380 случаев на 100 тыс. населения [42].Анализ показателей травматизма в России вы-явил, что 86,6% травматических повреждений кос-тей были переломами конечностей, в том числе 38,2% -верхних [1]. Значительную распростра-ненность травм в России А.Н. Коновалов (2008) связывал как с социально-экономическими изме-нениями в обществе, так и с сокращением целена-правленной работы по профилактике несчастных случаев, а также со снижением объема специаль-ных научных исследований по этой проблеме [8].Несмотря на то, что первое десятилетие XXI в. Всемирная организация здравоохранения провозгласила декадой по лечению костей и сус-тавов, определяя профилактику и лечение травм конечностей как одну из 5 приоритетных про-блем костно-мышечных заболеваний [95], до сих пор нет разработанного общепринятого прото-кола ведения больных с травмами ДФП, и мне-ния специалистов нередко диаметрально расхо-дятся [33, 98, 103].Недавний опрос кистевых хирургов разных стран показал, что существует большой разброс в выборе метода лечения при каждом конкрет-ном типе отчленения ДФП [103].
