Objective: The aims of our study are to identify the prevalence of LEOPAD in type 2 diabetes, to study the predictive factors of LEOPAD and to demonstrate the importance of establishing a specialized screening consultation in diabetic patients with LEOPAD and with high cardiovascular risk. Research design and methods: We carried out a prospective and descriptive study. The study involved 150 patients with type 2 diabetes who were followed on a regular basis. The different epidemiological, clinical and biological data were collected patient’s medical file. Each patient was investigated for cardiovascular risk factors. A thorough physical examination was performed in addition to biological assessment (fasting plasma glucose, HbA1c, lipid profile). After consent had been obtained, all patients underwent the ankle-brachial systolic pressure index (ABI) test. Results: The mean age of our patients was 57.46 ± 8.04 of the 150 patients, 24.7% had abnormal ABI suggestive of lower limb ischemia. Among them, 8.7% had an ABI of 1, 3, a presumed marker of vascular stiffness. The mean age of the LEOPAD population was 58 years. LEOPAD was predominantly asymptomatic in our population (88.1%). Only 5 patients (11.9%) had intermittent claudication. The comparative study of patients with LEOPAD showed a statistically conversely significant correlation between LEOPAD and lower levels of HDL. However, there is no statistically significant correlation with the following factors: smoking, sex, hypertension, dyslipidemia, obesity or overweight android pattern, the presence of microalbuminuria, renal failure and sedentary lifestyle. In terms of glycemic control, it did not show a statistically significant difference between the two groups. Moreover, it was noted that the patients with LEOPAD had had more micro vascular complications compared to patients without LEOPAD. Conclusion: Lower extremity occlusive peripheral artery disease occurred more commonly than it was expected in the diabetic population, associated with a high cardiovascular comorbidity. The sensitivity, specificity and simplicity of the ABI measurement promote its routine use and integration into the clinical approach of high-risk LEOPAD patients, especially the diabetics.

Diabetes constitutes a major public health concern, especially in developing countries, due to its frequency and severity related to the macroangiopathy complications. This study aimed at estimating the prevalence of macroangiopathy in type 2 diabetic outpatients attending the Antidiabetic Center of Abidjan (ADCA) and identifying the associated factors. It was a retrospective and analytic study that was conducted in 2014 at the ADCA. In total, 377 diabetic outpatients were included in the study. Patients had a mean age of 61.3 (SD: 8.9) years and were suffering diabetes for a mean duration of 11.8 years (SD: 5.6). In 95.5% of the cases, at least one cardiovascular disease risk factor was associated to diabetes. It was high level of cholesterol (37.1%), high level of Low Density Lipoprotein (LDL) cholesterol (72.4%), low level of High Density Lipoprotein (HDL) cholesterol (20.9%), high level of triglycerides (10.3%), hypertension (67.4%), and obesity (18.8%). Macroangiopathy was diagnosed in 62 out of 377 patients, meaning 16.4%. It was a coronary disease (44/377 meaning 11.7%), vascular cerebral accident (11/377 meaning 2.9%) and peripheral arterial disease of the lower limbs (10/377 meaning 2.7%). Diabetic patients with hypertension were more likely to have a macroangiopathy as compared to those without hypertension (p = 0.024; OR2.57, IC = [1. 13-5.85]). In addition, patients presenting a low level of HDL cholesterol were likely to have a coronary heart disease as compared to those with a normal level of HDL cholesterol (p = 0.008); OR = 2.72, IC [1. 28-5.76]. With regard to the severity of macroangiopathy complications, it is tremendous to reinforce the medical management of cardiovascular disease risk factors in type 2 diabetes patients in Ivory Coast.

Background. Type 2 diabetes mellitus (T2DM) is an important risk factor for peripheral artery disease (PAD). Measuring the Ankle-Brachial Index (ABI) in patients with diabetes is a simple way to screen for PAD. In the last decade, pathological ABI values (≤ 0.9; ≥1.3) were found associated with a higher cardiovascular (CV) risk and mortality. The role of classic risk factors for PAD in patients with diabetes is still unclear, and there is no specific PAD risk scoring system for these patients. The main goals of this study were to establish the prevalence of PAD in a T2DM population, and assess the relationship between PAD and the CV risk calculated with the CUORE Project score (CPS) (https://www.cuore.iss.it/). The association between the ABI, the main risk factors for PAD and T2DM complications was also investigated. Methods. Two hundred patients attending the Diabetes Clinic run by the ULSS 6 District Health Unit in Padua (north-east Italy) were consecutively enrolled, and their left and right ABI were measured. Twenty-seven patients with a history of CV events were excluded for the purpose of calculating the CPS, which was thus recorded for a total of 173 patients. Results. The prevalence of PAD in this study population was 17%. The CV risk calculated with the CPS tended to be higher (p = 0.0712) in the group with a pathological ABI than in the group with a normal ABI (0.91–1.29). The distribution of patients in the different CV risk classes did not differ significantly (p = 0.2208) between the two groups (pathological vs normal ABI). Glycated hemoglobin (r = -0.1591; p = 0.0244), total cholesterol (r = -0.1958; p = 0.0054), LDL cholesterol (r = -0.1708; p = 0.0156) and systolic blood pressure (r = -0.1523; p = 0.0313) correlated significantly and inversely with the left ABI. The frequency of diabetic retinopathy differed significant (p = 0.0316) between the two groups (pathological vs normal ABI), and was higher in the group with a pathological ABI. Conclusions. The data collected reveal a high prevalence of PAD in patients with T2DM, consistently with studies conducted in other countries. The CPS confirmed that patients with a pathological ABI have tendency to a higher CV risk. Overall, the present results point to the importance of an accurate CV assessment – also measuring individuals’ ABI and calculating their CPS - to better pinpoint those at high risk of PAD, especially among patients with T2DM.
