Among other uses, metallic silver and silver salts have currently been applied as antimicrobial agents in many aspects of medical industries, such as coating of catheters, dental resin composites and burn wounds, as well as in homeopathic medicine, with a minimal risk of toxicity in humans. However, their use in animal feeding as prebiotics have remain minimised, mostly because of the low cost antibiotics used as growth promoters in the second half of the XX Century. However, after the ban of this practice in the European Community, silver compounds appear as a potential alternative to other already in use, such as organic acids, oligosaccharides, plant extracts, etc. The major concerns about the safe use of an additive in animal feeding are its effective role as antimicrobial, acting selectively over potential pathogens but not over symbiotic microbial communities; a low toxic effect over the animal and its human consumer; and a low risk of environmental pollution. Metallic silver nanoparticles (up to 100 nm) allow for a higher antimicrobial effect than silver salts, are more resistant to deactivation by gastric acids and have a low absorption rate through the intestinal mucosa, thus minimising its potential risk of toxicity. Besides, it has been shown that the doses that promote animal physiological and productive effects are very low (20 to 40 ppm), especially compared to the 10 to 100-fold higher concentration used with other metallic compounds such as copper and zinc, thus precluding a harmful environmental effect. This chapter describes the reasons why silver nanoparticles could be applied to animal feeding, and provides with some available data in this regard. In any case, its registration as feed additive is a previous requisite before being applied in practical conditions.

Stigma, deforming metaphors and patients' moral experience of multibacillary leprosy in Sobral, Ceará State, Brazil

In response to the call for a new Science of Stigma, this anthropological study investigates the moral experience of patients diagnosed with severe multibacillary leprosy. From 2003 to 2006, fieldwork was conducted in the so-called "United-States-of-Sobral", in Ceará State, Northeast Brazil. Sobral is highly endemic for leprosy, despite intensified eradication efforts and a 30% increase in primary care coverage since 1999. Of 329 active leprosy cases at two public clinics, 279 multibacillary patients were identified and six information-rich cases selected for in-depth ethnographic analysis, utilizing illness narratives, key-informant interviews, home visits, participant-observation of clinical consultations and semi-structured interviews with physicians. A "contextualized semantic interpretation" revealed four leprosy metaphors: a repulsive rat's disease, a racist skin rash, a biblical curse and lethal leukemia. Far from value-free pathology, the disease is imbued with moral significance. Patients' multivocalic illness constructions contest physicians' disease discourse. "Skin Spot Day" discriminates more than educates. Patients' "non-compliance" with effective multi-drug therapy is due to demoralizing stigma more than a rejection of care. "Social leprosy" in Northeast Brazil deforms patients' moral reputations and personal dignity.

The antimicrobial properties of engineered silver nanoparticles (AgNPs) have led to their wide use in diverse consumer products. Ampicillin too, acts as a broad-spectrum antibiotic and thus is prescribed for the treatment of many common infections, but with the problematic emergence of ampicillin-resistant bacteria. As a consequence, there has been some interest in the combination of these two distinct chemistries prompted by the clinical challenge of resistance. Prior to trials of combination therapy, however, it is important to understand the impact on human microbiomes. Here we investigated the effect of ampicillin and AgNPs, both individually and in a combined therapy on a human intestinal ecosystem known as a defined experimental community (DEC-60). The DEC-60 consortia was co-treated with a concentration of AgNPs (50 mg/L) known to have a minimal impact, and a broad range of the antibiotic up to the clinical dose (6 mg/L). The addition of AgNPs to sub-clinical doses of ampicillin (0.06 and 0.6 mg/L) had a significantly impact (p<0.001-0.05) on gas production (%CO 2 and %N 2 ) and changed the composition of fatty acid methyl ester signatures so that they were distinct from each individual antimicrobial, as well as un-treated control cultures. DNA sequencing, supported by multidimensional scaling analysis, confirmed the community shift and showed distinct phylogenetic distributions at different concentrations of ampicillin, depending on the presence of AgNPs. Together, these results suggest that the antibacterial efficacy of sub-clinical doses of ampicillin was increased by AgNPs, possibly due to the synergistic effect of damage to the bacterial cell walls. Not only does this analysis provide insight into AgNP toxicity, it offers some promise that combined antimicrobial therapies may have value in a clinical setting.
