Are preoperative histology and MRI useful for classification of endometrial cancer risk?

Abstract: BackgroundThe 2010 guidelines of the French National Cancer Institute (INCa) classify patients with endometrial cancer into three risk groups for lymph node invasion and recurrence on the basis of MRI and histological analysis of an endometrial specimen obtained preoperatively. The classification guides therapeutic choices, which may include pelvic and/or para-aortic lymphadenectomy. The purpose of this study was to evaluate the diagnostic performance of preoperative assessment to help identify intermediate- o… Show more

“…The first objective of this study was to correlate the grade and stage based on preoperative assessment with that of the final pathology. Our study results correlated with those of previous studies [ 13 15 ]. Body et al showed that preoperative assessment had a 90% negative-predictive value for the diagnosis of grade 3 tumors [ 15 ].…”

“…Our study results correlated with those of previous studies [ 13 15 ]. Body et al showed that preoperative assessment had a 90% negative-predictive value for the diagnosis of grade 3 tumors [ 15 ]. Batista et al suggested that preoperative endometrial sampling was found to be a modest overall predictor of postoperative histological grading [ 14 ].…”

“…Based on title and abstract, 525 studies were relevant. After full‐text screening, 83 studies, comprising 18,205 patients, were included in the systematic review (supplemental online Appendix 2) . Three studies lacked raw data, and because the authors did not respond after being contacted, these publications were excluded .…”

“…EC risk stratification is currently performed using clinicopathological data (stage, grade, MI and lymphovascular invasion), which are not always representative of the tumor biology ( 32 ) and show discrepancies in the final diagnosis ( 33 35 ). Therefore, there is a need to identify EC biomarkers that contribute to the prediction of the early evolution of tumors, particularly in stage I tumors that may relapse, with the aim to optimize preoperative disease management and follow-up.…”
