Bone loss (osteopenia) is a common complication in human solid tumour. In addition, after surgical treatment of gynaecological tumour, osteoporosis often occurs due to the withdrawal of oestrogen. The major characteristic of osteoporosis is the low bone mass with micro‐architectural deteriorated bone tissue. And the main cause is the overactivation of osteoclastogenesis, which is one of the most important therapeutic targets. Inflammation could induce the interaction of RANKL/RANK, which is the promoter of osteoclastogenesis. Triptolide is derived from the traditional Chinese herb lei gong teng, presented multiple biological effects, including anti‐cancer, anti‐inflammation and immunosuppression. We hypothesized that triptolide could inhibits osteoclastogenesis by suppressing inflammation activation. In this study, we confirmed that triptolide could suppress RANKL‐induced osteoclastogenesis in bone marrow mononuclear cells (BMMCs) and RAW264.7 cells and inhibited the osteoclast bone resorption functions. PI3K‐AKT‐NFATc1 pathway is one of the most important downstream pathways of RANKL‐induced osteogenesis. The experiments in vitro indicated that triptolide suppresses the activation of PI3K‐AKT‐NFATc1 pathway and the target point located at the upstream of AKT because both NFATc1 overexpression and AKT phosphorylation could ameliorate the triptolide suppression effects. The expression of MDM2 was elevated, which demonstrated the MDM‐p53‐induced cell death might contribute to the osteoclastogenesis suppression. Ovariectomy‐induced bone loss and inflammation activation were also found to be ameliorated in the experiments in vivo. In summary, the new effect of anti‐cancer drug triptolide was demonstrated to be anti‐osteoclastogenesis, and we demonstrated triptolide might be a promising therapy for bone loss caused by tumour.

Platelet-rich plasma (PRP) is a blood component with a supraphysiologic concentration of platelets, usually obtained from the processing of autologous blood by different techniques, such as centrifugation and filtering. Despite the fact that strict recommendations on the most adequate techniques that should be employed for the production of PRP are still lacking, it is logical to assume that the type of method used for blood processing may determine the resulting concentration of platelet and other components, possibly impacting on its therapeutic potential. 1 The physiological rationale of the non-transfusion use of PRP relies on the high concentration of growth factors contained in platelets, such as the platelet derived-growth factor, the vascular endothelial growth factor and the fibroblast growth factor. 2 Several biological functions are attributed to these growth factors, including the proliferation of epithelial and mesenchymal cells, the stimulation of myoblasts and fibroblasts and angiogenesis. 3-5 Abstract Introduction: Platelet-rich plasma (PRP) is a blood component therapy with a supraphysiological concentration of platelets derived from allogenic or, more commonly, autologous blood. PRP has been used in different non-transfusion indications because of its role in the promotion of tissue repair and healing, in fields such as Traumatology, Dermatology and Dentistry.Objective: To provide a synthesis of the efficacy of PRP for different clinical situations. Methods: Systematic searches were carried out in MEDLINE, Embase, CochraneLibrary and LILACS in July 2018 to identify systematic reviews (SRs) of randomized clinical trials (RCTs) focusing on PRP for non-transfusion use. Two authors independently screened all retrieved references in two stages (titles and abstracts at a first stage and full texts at a second stage). The methodological quality of SRs that met the eligibility criteria was appraised by AMSTAR 2. Conclusions were based on the most recent SRs with highest quality. Results:One thousand two hundred and forty references were retrieved. After checking the inclusion criteria, 29 SRs of RCTs related to three different fields (wound care, Orthopedics and Dentistry) were included. The results suggest the benefit of PRP for different clinical situations, such as diabetic wounds, acute lesions of musculoskeletal system, rotator cuff lesions, tendinopathies, knee and hip osteoarthritis, total knee arthroplasty, allogenic bone graft for dental implants and periodontal intrabony defects. Conclusion:There is low to moderate quality evidence supporting the efficacy of PRP for specific clinical situations. The low quality of the evidence limits the certainty of these findings. Well-planned and well-conducted RCTs are still needed to further assess the efficacy of PRP.

Abstract:The poor healing potential of tendons is still a clinical problem, and the use of Platelet Rich Plasma (PRP) was hypothesized to stimulate healing. As the efficacy of PRPs remains unproven, platelet lysate (PL) could be an alternative with its main advantages of storage and characterization before use. Five different blood products were prepared from 16 male donors: human serum, two PRPs (Arthrex, (PRP-ACP); RegenLab (PRP-BCT)), platelet concentrate (apheresis, PC), and PL (freezing-thawing destruction of PC). Additionally, ten commercial allogenic PLs (AlloPL) from pooled donors were tested. The highest concentration of most growth factors was found in AlloPL, whereas the release of growth factors lasted longer in the other products. PRP-ACP, PRP-BCT, and PC significantly increased cell viability of human tenocyte-like cells, whereas PC and AlloPL increased Col1A1 expression and PRP-BCT increased Col3A1 expression. MMP-1, IL-1β, and HGF expression was significantly increased and Scleraxis expression decreased by most blood products. COX1 expression significantly decreased by PC and AlloPL. No clear positive effects on tendon cell biology could be shown, which might partially explain the weak outcome results in clinical practice. Pooled PL seemed to have the most beneficial effects and might be the future in using blood products for tendon tissue regeneration.
