Are functional capacity evaluations affected by the patient’s pain?

Abstract: Functional capacity evaluations (FCE) are comprehensive batteries of performance-based tests used commonly to inform return-to-work decisions for injured workers. As many people undergoing FCE have painful musculoskeletal conditions limiting their work ability, pain becomes a critical factor in the assessment of function. This paper considers the available literature related to the influence of pain on FCE, which clearly indicates FCEs are behavioral assessments influenced by pain intensity and other pain-rela… Show more

“…In evaluating a client's response to physical challenges, FCE evaluations essentially use behavioral performance as a proxy for work capability. 64 However, we must guard against the "illusion of objectivity" that functional measurements are somehow unaffected by the client's interpretations of pain and disability. Any interpretation of such behavior must be integrated with an appraisal of the person's clinical history, beliefs, and emotional responses.…”

“…A patient's performance is determined by biological, psychological and social factors [4]. Pain related fear, also referred to as fear avoidance beliefs or fear of movement and reinjury [5], and pain [6] have been identified as examples of psychological factors influencing a patient's performance during an FCE.…”

“…In a recent review pain intensity has been identified to consistently associate negatively with patient performances during FCEs [6]. The strength of the significant associations between pain intensity and FCE performances in patients with chronic pain vary from r  = −0.25 to r  = −0.56 [6].…”

“…These Wndings suggest that the use of opioids for the management of occupational LBP may not always achieve its objectives. In fact, the relationship between pain intensity and functional capacity in people with LBP is poorly understood (Gross 2006) and therefore needs to be further investigated.…”
