Arborizing vessels in a targetoid hemosiderotic hemangioma: mistaken dermoscopic diagnosis of basal cell carcinoma

Abstract: Targetoid hemosiderotic hemangioma (THH) or hobnail hemangioma (HH) is a benign vascular lesion that presents with the classical clinical presentation of a ring-shaped tumor having a targetoid appearance, with a central purple-brown papule surrounded by a thin pale area and an ecchymotic ring on the outside. Dermoscopic features and patterns of HH have been documented and have proven to be sufficient to establish a clinical diagnosis in many cases. We present a facial lesion in which both the clinical pr… Show more

“…The effi cacy of dermatoscopy (also known as dermoscopy) to enhance the diagnosis of vascular skin lesions is well reported and reviewed. [1][2][3][4][5][6][7][8] While performing dermatoscopeguided surgical procedures [9][10][11] we were alerted to the ease of identifying the vascular lesions and the vasculatures of lesions that were not primarily vascular diseases. However, to our best knowledge, a systematic and quantitative study to evaluate the effi cacy of dermatoscopy in vascular skin lesions has not been reported.…”

“…A series of dermoscopic vascular features were selected to describe the change in vascular structures by HRVD under OP treatment. This selection was done by taking into account both the dermoscopic descriptions of vascular lesions 32‐34,36‐39 and the current nomenclature concerning the morphology of vessels in general skin neoformations 30,40,41 . After HRVD of the whole IH structure, a prevalent dermoscopic pattern was assessed if present in ≥70% of the lesion.…”

“…[ 4 ] The tumor can stain positive for both lymphatic (D 2-40) and vascular markers (CD34) and hence its origin is controversial, with some suggesting a vascular origin while others proposing a lymphatic origin. [ 5 ] The diagnosis is more of academic importance since the management is excisional biopsy, similar to that of any other typical conjunctival hemangioma.…”
