Aqueous Vascular Endothelial Growth Factor Levels Are Associated With Serous Macular Detachment Secondary to Branch Retinal Vein Occlusion

Abstract: Our results indicate that excessive increase in vascular permeability because of VEGF upregulation may contribute to the development of SRD secondary to branch retinal vein occlusion.

“…Several authors have reported that the incidence of SRF among eyes with major BRVO was significantly higher than that among eyes with macular BRVO [15,16], and that the mean vascular leaking area in the retina was larger in BRVO patients with SRF than in patients without SRF [15]. Park et al also showed that the mean aqueous levels of VEGF were significantly higher in BRVO eyes with SRF than in eyes with cystoid ME without additional SRF [15] . Baseline VA of patients with and without SRF in BRVO and CRVO patients did not differ significantly in our study.…”

“…Some authors have postulated an insufficient draining function of the RPE, which might be due to damage to the RPE cells or a very fast fluid accumulation that exceeds the fluid transport capacity of the retinal pigment epithelium [11,14]. “…[3][4][5][6][7] Several recent studies have reported that VEGF has an important role in the pathophysiology of ME by inducing blood-retinal barrier breakdown and increasing vascular permeability. [6][7][8][9] Elevated VEGF level in vitreous has been reported in patients with BRVO, and this elevation was correlated with the severity of ME and area of non-perfusion. 7 These correlations prompted interest in the use of pharmacologic inhibition of VEGF for treatment of ME.…”

“…In general, the data indicate that levels of VEGF in the aqueous are lower than that in the vitreous. Control levels of VEGF measured in subjects who had surgery to remove cataracts ranged 50-112 pg/ml [40,45]. The highest levels of VEGF were found in the aqueous of patients with active disease such as wet age-related macular degeneration (AMD), PDR, and diffuse macular edema (DME).…”

“…In 6 of the studies, samples were obtained from patients who were already undergoing minimally invasive intravitreal injections of bevacizumab (Avastin, Genentech, South San Francisco, CA) or triamcinolone [40,[42][43][44][45][46]. Remarkably, none of the studies in this survey involved subjects who were injected with ranibizumab or pegaptanib.…”
