Aquarium Dermatitis: Cercarial Dermatitis in an Aquarist

Abstract: A 33-year-old man presented with very itchy red papules on the back of his hands and forearms. These papules appeared about 90 min after he had cleaned his aquarium in which he kept native fish and watersnails. He had obtained the watersnails some weeks before from a nearby pond. Examination of water from the aquarium revealed cercariae. The clinical diagnosis of cercarial dermatitis was corroborated. Cercarial dermatitis has repeatedly been seen in swimmers but not in aquarists keeping fish in a home aquarium. Show more

“…Besides bilharziasis cutanea tarda, cercarial dermatitis and ‘Katayama fever’ are more common cutaneous manifestations of schistosomiasis. An itchy maculopapular rash after repeated exposure to cercariae is well-known hypersensitivity skin reaction also named ‘swimmer’s itch’ [13]. However, this reaction is mainly induced by avian cercariae penetrating human skin without further progression of their life cycle.…”

“…However, because studies on humans are rarely performed (112,225,226), a mouse model was established to provide more-detailed studies on immunohistopathology. Primary infection of mice with T. regenti causes an acute inflammatory reaction with edema and vasodilatation (227).…”

“…In conclusion, we report a biology teacher who, only a few hours after cleaning the school aquarium, developed a monomorphic eruption on the areas of skin exposed to the aquarium water (hands, forearms). To our knowledge, this is the second published case of the development of cercarial dermatitis after cleaning an aquarium 7 …”

“…Cercarial dermatitis is characterized by an itchy monomorphic eruption caused by an immune reaction towards the penetration of cercariae into the skin 2–7 . During late summer, cercarial dermatitis is a relatively common disease in Schleswig‐Holstein due to the habit of many people swimming in the lakes of this region.…”
