Appropriate Use of Dopamine Agonists and Levodopa in Restless Legs Syndrome in an Ambulatory Care Setting

Abstract: The findings of this study confirm the need for provider education about the appropriate use of levodopa and dopamine agonists in patients with RLS. Appropriate use of these drugs may help decrease unnecessary adverse effects, complications, and costs.

“…The dopamine precursor levodopa was used for years for RLS treatment until the newer dopamine agonists were introduced. Levodopa is effective in improving RLS symptoms, but rebound (worsening of symptoms in the early morning hours) and augmentation (earlier onset of symptoms in the evening or afternoon, increase in symptoms, and spread of symptoms to involve other extremities) can occur in up to 82% of patients (Allen & Earley, 1996;Molokwu, 2008;Stiasny, Robbecke, Schuler, & Oertel, 2000).…”

“…Dopamine agonists are the drugs of choice for treatment of RLS, largely due to their ability to relieve moderate-to-severe RLS symptoms with fewer side effects than the precursors (Baker, White, & Coleman, 2008;Garcia-Borreguero et al, 2007;McCormack & Siddiqui, 2007;Molokwu, 2008;Trenkwalder, 2006). These medications produce dopaminelike effects at the actual receptor sites (subtypes D 1 through D 3 ).…”

“…Carbidopa/levodopa and dopamine agonists (pramipexole, ropinirole) are 2 commonly used dopaminergic medications in adult populations. Ropinirole was the first Food and Drug Administration (FDA)-approved medication for treatment of moderate to severe primary RLS as of May 2005 37. Subsequently, pramipexole was approved for the same indication in November 2006.…”
