This study examines the available evidence of high reliability organisational (HRO) theory as a strategy to manage construction safety: (1) Background: High reliability organisations (HROs) have been under investigation by organisational scholars to understand how they function at an exceptionally high level with few or no accidents under challenging circumstances. The construction industry is a high risk industry and is also known for a high fatality rate around the world. This systematic review examines the available evidence of HROs as a strategy to manage construction safety; (2) Methods: A systematic review to summarise and critically appraise the literature on high reliability organisational theory, aimed at improving construction safety; (3) Results: Of 2724 articles found, fifteen studies met the inclusion criteria for qualitative synthesis and review. Six of the studies were from construction, four were from general HROs research, two were from health care, and three were from the aerospace, oil and gas, and nuclear industries; (4) Conclusion: Based on the available evidence, transferring the practices and principles of HROs to construction, the validation of proposed assessing tools and a consensus HRO definitions are the major issues identified.

A construction system is an organizational and technological system that produces buildings or civil engineering structures for human use by fabricating and installing materials into an integrated structure according to design specifications. Failures of such construction systems materialize in various forms, including defective design, rework, and dysfunctional building systems. Frequent accidents and building defects in the construction sector clearly indicate that our society still needs to improve the safety of construction systems. Research studies on how major accidents and minor defects occur are often conducted in isolation by different researchers, and existing system safety concepts and frameworks are not necessarily suited to handling the dynamic nature of temporary multiple organizations (TMOs)—a feature that characterizes the uniqueness of construction projects. We need a framework that can better describe failures specific to construction, and capture the wide spectrum of these failures. Here, we build on concepts from system safety research to develop a graphical notation with “frames” and system hierarchy as the main components to illustrate how pathogens are generated and propagated by defective processes and end up embedded in the physical structure.

It is well documented that farming is a high-risk industry in terms of fatalities and injuries, and with numerous risk factors associated with operating the farm. It has also proved difficult to find evidence for the effectiveness of interventions. Moreover, farming is in transition, with ongoing technological transformations as well becoming increasingly more globalized. Thus, new perspectives that allow for more systemic understandings in the management and promotion of occupational health and safety (OHS) are needed. Our main objective is to present an integrated theoretical understanding of the farm as an enterprise and an integrated element in the political-economic agricultural system. The main question is how can farmers organize and manage the farm, in order to simultaneously improve efficiency, quality and OHS based on systemic models for OHS and a systemic understanding of the political-economical system of Norwegian agriculture? The framework is adapted to the Norwegian agricultural context, with ongoing transformations both technologically and organizationally, including visions and plans set by Norwegian agriculture itself. However, the framework can be applied irrespective of national context.
