Introducción: El tratamiento del tumor de fosa posterior implica la resección quirúrgica, sola o en combinación con tratamientos coadyuvantes, como la radioterapia y quimioterapia. Luego de la resección quirúrgica, el 20-30% de los niños presentan síndrome de fosa posterior. En esta serie de casos, describimos el abordaje médico y la recuperación funcional de tres niños con síndrome de fosa posterior en una clínica de rehabilitación de Buenos Aires. Presentación del caso: Se analizaron tres niños (dos de sexo masculino y una femenino), de entre 5 y 15 años de edad, con síndrome de fosa posterior dentro de los 30 días postquirúrgicos e internados para neurorrehabilitación entre el 2016 y 2021. Se realizaron evaluaciones, previo y posterior a la rehabilitación, de los hallazgos en los estudios de imágenes, de las manifestaciones neurológicas mediante la escala para la evaluación y calificación de la ataxia (SARA) y de la movilidad funcional mediante la escala de movilidad funcional (FMS). Conclusión: Al alta del centro de rehabilitación, los tres niños evidenciaron mejoría en las manifestaciones neurológicas, la movilidad funcional y los hallazgos en las imágenes. Sugerimos la realización de estudios prospectivos con mayor tamaño muestral y utilizando escalas validadas.

Zusammenfassung Hintergrund/Ziel Die Scale for the Assessment and Rating of Ataxia (SARA) ist ein praxistaugliches Assessment für die Einschätzung des Schweregrades einer Ataxie und für die Evaluierung von Therapiemaßnahmen. Um im deutschsprachigen Raum über eine entsprechend internationalen Vorgaben übersetzte Version zu verfügen, war das Ziel dieser Arbeit, die SARA ins Deutsche zu übersetzen und für den deutschsprachigen Raum kulturell anzupassen. Methode Der Übersetzungsprozess beinhaltete 6 Schritte. Dabei wurde die Verständlichkeit der Übersetzung in Interviews mit den späteren Nutzer*innen überprüft. Ergebnisse Neun Physiotherapeut*innen und sechs Ärzt*innen mit unterschiedlichen Arbeitsumgebungen wurden interviewt. Sieben Personen waren in Deutschland und je vier in Österreich bzw. der Schweiz tätig. Die Interviews führten zu einer Präzisierung der übersetzten Version. Im länderspezifischen Vergleich wurden keine Auffälligkeiten der Verständlichkeit festgestellt. Schlussfolgerung Mit dieser Arbeit ist eine von der Mitautorin der Originalpublikation autorisierte deutsche Version der SARA verfügbar. Die Ergebnisse liefern außerdem methodische Erkenntnisse zum Übersetzungsprozess von beobachtungsbasierten standardisierten Assessments.

Background/objectiveIn children, medulloblastoma (MB) is the most prevalent posterior fossa tumor. The first line of treatment is maximal safe resection. Therefore, symptoms of ataxia are commonly seen. Training the brain on balance and cognitive tasks makes balance more automatic than without cognitive tasks. The goal was to assess the effectiveness of dual‐task practice on balance after MB excision in children with ataxia.MethodsThirty children with ataxia after MB resection at Children Cancer Hospital Egypt were randomized into two equal groups. Exercises for mobility, balance, and gait training were given to both groups. The research group underwent a specific dual‐task program (balance and cognitive). The program ran 3 days per week for 8 weeks. Children were evaluated before and after the treatment regimen using the Scale of Assessment and Rating of Ataxia (SARA), the HUMAC Balance System, Pediatric Balance Scale, and Functional Independent Measurement. All children's legal guardians signed an ethical agreement.ResultsA notable improvement in balance was found in the dual group in Pediatric Balance Scale (PBS) (p = .028) and stability test (p = .0001) in favor of the study group. No discernible difference was observed in the Functional Independent Measurement score among the two groups (p = .158), although there was a statistically significant increase in both groups after treatment.ConclusionDual‐task program is more effective than traditional physical therapy alone in improving balance in children with ataxia after MB resection.
