Application of Orthopedic Principles in Evaluation of the Temporomandibular Joint

Abstract: Temporomandibular joint dysfunction is often the cause of a variety of symptoms throughout the head and neck. Because such dysfunction plays an integral part in head pain and earache, differential diagnosis should include an evaluation of the temporomandibular joint. Dentists are the primary professionals involved in temporomandibular joint evaluation and treatment, but they are mainly concerned with nonmovable radio-opaque parts. Physical therapists, then, must learn more about this joint so they can assist d… Show more

“…1 During clinical examination, limitation of maximum mouth opening (MMO) is considered an important sign of a possible diagnosis of TMD. [2][3][4][5] Unfortunately, the high variability of normal MMO measurement makes the evaluation more difficult. In fact, values from 32 mm to 77 mm have been reported in the literature for adults.…”

“…The dislocation of the temporomandibular joint has been suggested to be the result of imbalance in the jaw-opening muscles, or the spasm of the jaw-opening muscles on the side opposite to the deviation [12]. Dislocations usually treated by open reduction and immobilizing of the joint after the procedure.…”

“…However, findings of anamnestic surveys should not be overrated in general because of the lack of accordance with clinical diagnoses [6,26]. Both the metric measurements of jaw movements [8,9,12] and orthopedic examination techniques of the Manual Functional Analysis seem to be reproducible and objective [10,15,18,24,27,40,41].…”

“…Jedoch dürfen die Ergebnisse anamnestischer Befragungen grundsätzlich nicht überbewertet werden, da sie häufig nicht mit den klinischen Befunden übereinstim-men [6,26]. Sowohl eine metrische Messung der Unterkieferbewegungen [8,9,12] als auch die orthopädischen Untersuchungstechniken der Manuellen Funktionsanalyse scheinen reproduzierbar und objektiv [10,15,18,24,27,40,41].…”
