Application of failure mode and effects analysis (FMEA) to pretreatment phases in tomotherapy

Abstract: The aim of this paper was the application of the failure mode and effects analysis (FMEA) approach to assess the risks for patients undergoing radiotherapy treatments performed by means of a helical tomotherapy unit. FMEA was applied to the preplanning imaging, volume determination, and treatment planning stages of the tomotherapy process and consisted of three steps: 1) identification of the involved subprocesses; 2) identification and ranking of the potential failure modes, together with their causes and eff… Show more

“…Second, many reported radiation therapy incidences resulted from incorrect or inappropriate use of radiation treatment devices due to miscommunication or misunderstanding rather than device failures. As such, the development of quality management programs that focuses on the design and execution of various radiotherapy processes has become a subject of significant interest in recent years 12, 13, 14, 15, 16, 17, 18, 19, 20. Following the recommendations from the AAPM task group report No.…”

“…Data have been published on FMEA for stereotactic radiosurgery (SRS),8, 9, 10 stereotactic body radiation therapy (SBRT),11, 12 tomotherapy,13 intraoperative radiation therapy (IORT),14 treatment planning,15 and dynamic MLC tracking 16. These valuable publications primarily come from large institutions where staffing levels may allow for variable amounts of time allocation for projects of this magnitude.…”

“…marie.cantone@unimi.it 1 http://www.accuray.com/treatment-centers In a recent study, the FMEA approach was applied to the pretreatment phases of the tomotherapy process implemented at the San Raffaele Scientific Institute (Broggi et al, 2013). The results of this investigation allowed the improvement of the local safety measures with the adoption of novel solutions for dealing with the failures of greatest concern in terms of overall risk or severity of the consequences.…”
