Applicability and midterm results of branch cuff closure with vascular plug in branched endovascular repair for thoracoabdominal aortic aneurysms

Abstract: Closure of the branch cuff of multibranched stent graft with Amplatzer Vascular Plug is feasible and effective. It was not associated with adverse aneurysm outcomes, and it is very useful especially when using an off-the-shelf device in the acute setting.

“…Closure of an unused branch cuff with a vascular plug effectively prevents a cuff related endoleak in the presence of an occluded TV or failure of TV catheterisation. 16 Low profile stent grafts increase the applicability of b-EVAR with comparable outcomes to the standard profile version. 7,17 Striving for good long-term results has changed some of the authors' practices.…”

“…This technique has been described previously and also works fine during long term follow up. 6 This technique is useful to adapt the T Branch to current anatomy but is also a bailout maneuvre to avoid an endoleak when a specific target vessel cannot be catheterized. 6 At present, endovascular thoracoabdominal repair does imply 3-6 hours of operation before the aneurysm is excluded from flow, so it is primarily suitable for stable patients.…”

“…1 A recent study by Hongku and colleagues 2 presented a series of 17 patients in whom an Amplatzer plug was used to embolize unused branches during branched endovascular aortic repair for thoracoabdominal aortic aneurysms. 2 Others have described the use of the Amplatzer septal occluder device to close the neck of thoracic aortic pseudoaneurysms, in both the ascending and descending thoracic aorta. [3][4][5][6] This case demonstrates another adaptation of the Amplatzer vascular plug to treat a pseudoaneurysm originating from the limb of a branched thoracic aortic graft.…”
