Appendicular Biopsy in Total Colonic Aganglionosis: A Histologically Challenging and Inadvisable Practice.

Abstract: Intraoperative characterization of appendicular innervation as a guide to the diagnosis of TCA is unreliable. ; in part because of the possibility of skip segment disease / variable hypoganglionosis . We propose terminal ileal biopsies for diagnosis and leveling of aganglionosis. AChE on frozen / calretinin on paraffin tissue is the 'best approach' to avoid diagnostic errors.

“…A recent case report highlighted a rare instance of total colonic aganglionosis in a Caucasian infant, diagnosed retroactively [ 8 ]. Diagnosis based on histological findings from intestinal sections, ileum, and appendix biopsy samples remains a topic of debate among researchers [ 8 , 9 ]. Some researchers advocate for caution in the diagnosis, expressing reservations about Hirschsprung's disease, emphasizing the importance of consistent clinical presentations irrespective of radiological and biopsy findings [ 3 ].…”

“…Different points of view have been expressed in research papers of the past. While some researchers felt that the appendix was an unreliable tool in the diagnosis of TCA, [6][7][8][9] others concluded that the appendix was indeed useful in the diagnosis of TCA. [10][11][12][13] Diagnosis of TCA is challenging.…”
