The accurate diagnosis remains challenging in some cases despite advances in both minimally invasive surgery and radiology. Stump appendicitis is a rare complication after appendectomy. It is defined as the acute inflammation of the residual appendix. A small number of stump appendicitis cases have been reported. Case Report: We report a case of stump appendicitis in a 42-year-old female, nine months following a laparoscopic appendectomy. She presented with a 24-hour history of abdominal pain, which started periumbilically and then localized to the right lower quadrant. Physical examination showed tenderness in the right iliac fossa with evidence of rebound and guarding. Laboratory studies were remarkable for leukocytosis. Computed tomography scan of the abdomen and pelvis showed a remnant appendicular segment with a maximum cross diameter of about 1.2 cm, associated with local inflammatory changes and surrounding fat stranding. An open stump appendectomy was performed uneventfully. Conclusion: Stump appendicitis is a rare but serious complication of appendectomy. It can represent a diagnostic dilemma if the treating clinician is unfamiliar with this rare clinical entity. Prompt recognition is important to avoid serious complications. Proper identification of the appendicular base intraoperatively and leaving the appendix stump shorter than 5 mm decrease the risk of stump appendicitis.

ÖZETAkut apandisit, cerrahi akut karın nedenleri sıralandığında listenin en üst sıralarında yer almaktadır. Apendektomi, dünyada en yaygın uygulanan cerrahi prosedürdür. Güdük apandisit apendektomi sonrası nadir görülen komplikasyonlardan birisidir. İlk defa 1945 yılında Dr. Rose tarafından tanımlanmıştır. İnkomplet olarak yapılan apendektomi sonrası remnant apendiksin enflamasyonu ile karakterize bir tablodur. Kliniği diğer akut karın nedenlerine benzerdir. Tanıda anamnez ve fizik muayenenin yanında abdominal tomografi önemli yer tutmaktadır. Akut karın tablosu ile acil servise başvuran ve geçmişinde apendektomi öyküsü olan hastalarda mevcut ağrının özellikle sağ alt kadranda olması güdük apandisitin ayırıcı tanıda mutlaka akılda tutulmasını gerektirmektedir.Anahtar Sözcükler: Apandisit, güdük apandisit, apendektomi ABSTRACTAcute appendicitis is at the top of the list when the reasons of acute abdomen are listed. Appendectomy is the most commenly performed surgical procedure worldwide. Stump appendicitis is one of the rarely seen complications after appendectomy. It was first described by Dr. Rosein 1945. Stump appendicitis is a clinical condition characterized by the inflammation of remnant appendix after incomplete appendectomy. The clinical picture resembles the reasons causing acute abdomen. Abdominal CT has an important role in diagnosis as well as history taking and physical examination. In patients admitting to emergency department with acute abdomen who has previous appendectomy history,stump appendicitis must be considered in the differential diagnosis, especially when the pain is present at right lower quadrant.Key Words: Appedicitis, stumpappendicitis, appendectomy GİRİŞ Akut apandisit, cerrahi akut karın nedenleri sıralandığında listenin en üst sıralarında yer almaktadır. Apendektomi dünyada en yaygın uygulanan cerrahi prosedürdür. Bu kadar sık yapılan bir operasyon olması sebebiyle komplikasyonların çeşitli ve bir o kadar da sık olması kaçınılmazdır. Bu komplikasyonlar arasında yara yeri enfeksiyonu, intra-abdominal apse, peritonit, kanama, ileus ve daha nadir olarak görülen güdük apandisit sayılmaktadır (1). İlk defa 1945 yılında Dr. Rose tarafından tanımlanan güdük apandisit nadir görülen fakat hayatı tehdit eden ciddi komplikasyonlara neden olabilen bir durumdur (2). Bu tarihten itibaren literatürde61 vaka bildirilmiştir (3). Normal popülasyonda akut apandisitin görülme oranı insanların tüm hayatları boyunca yaklaşık %7 iken raporlanmış güdük apandisit ile karşılaşma insidansı 1/50000'dir (4). Fakat doğru insidansın daha yüksek olduğu tahmin edilmektedir (5). İnkomplet olarak yapılan apendektomi sonrası remnant apendiksin enflamasyonu ile karakterize bir tablodur. Apendektomi sonrası herhangi bir yaşta (11-72 yaş) ve sürede (2 ay -50 yıl) oluştuğu bildirilmiştir (6-8).
