Appendiceal Diverticulitis Presenting as Acute Appendicitis: A Case Report

Abstract: Appendiceal diverticulitis (AD) is an overlooked pathology that carries a high risk of perforation and associated neoplasia, especially carcinoid tumours and mucinous adenoma. AD may be congenital, but more commonly acquired. It may cause diverticulitis, which causes clinical and radiological signs like those of acute appendicitis, and that may delay the diagnosis till it is confirmed on histopathological examination. Here we report a case of acute AD in a case initially diagnosed as acute appendicitis.

“…AD is usually overlooked because of its rarity and resemblance to other ileocecal diseases (CD and AA) [ 12 , 13 ]. Clinical presentation: although AD presents with similar symptoms and signs to AA [ [14] , [15] , [16] ], some differences have been observed. Compared to AA clinical presentation AD has a longer duration of pain (1–14 days), a greater presence of RLQ abdominal pain, a lower frequency of nausea and vomiting and it develops mainly in adults [ 1 ].…”

“…Appendiceal diverticula are mostly discovered incidentally by imaging and/or intraoperatively by histological diagnosis. Despite the paucity of specialized literature on the subject, several studies consider appendicular diverticulosis as precancer (5)(6)(7)(8)(9).…”
