Appendiceal carcinoma associated with microsatellite instability

Abstract: Approximately 15% of colorectal cancer (CRC) cases exhibit microsatellite instability (MSI), which appears to be associated with unique biological behavior. The present study presents a case of appendiceal carcinoma associated with MSI that responded well to adjuvant chemotherapy. Clinical, pathological and immunohistochemical (IHC) characteristics have been described. The 60-year-old male patient had suffered from recurrent lower abdominal pain associated with abdominal distention for 6 months; then, followin… Show more

“…Co-mutations in other genes, such as the tumor suppressor gene SMAD4, are common in high-grade mucinous appendiceal adenocarcinomas, probably favoring the progression of indolent low-grade neoplasms into aggressive high-grade adenocarcinoma [53]. Some mucinous adenocarcinomas of the appendix can exhibit microsatellite instability [74]. In some instances, the appendix attaches itself to the surrounding organs or often fistulize internally inside the body, into the bladder, ileum, umbilicus, aorta, sig- moid colon, cecum, etc.…”

The association with genetic causes is not well known, with only a few cases reported in which mutations occurred in the Some mucinous adenocarcinomas of the appendix can exhibit microsatellite instability [74]. “…Microsatellite instability can manifest in both familial and sporadic colorectal cancer (CRC). Familial cases, comprising approximately 2-3% of all CRC cases [234], are associated with inherited mutations in DNA mismatch repair genes, namely, MLH1, MSH2, MSH6, or PMS2. In rare instances, a deletion in the last exon of the EPCAM gene can cause MSH2 methylation, resulting in heritable MSI.…”

“…9 The prevalence of the MSI-H/dMMR phenotype in appendiceal carcinomas is estimated to be 2.8%. 10 The 5-year survival for appendiceal carcinoma ranges from 20% to 60%. 10 Current standard of care for the treatment of small bowel adenocarcinoma is chemotherapy with fluorouracil-leucovorin-oxaliplatin (known as FOLFOX) or capecitabine-oxaliplatin (known as CAPOX).…”
