Appendectomy in a child with cyclic neutropenia in profound neutropenic episode

Abstract: Our experience in the treatment of a 4-year-old boy with cyclic neutropenia who was admitted for urgent appendectomy is described. The postoperative course was uneventful with high daily doses of granulocyte colony-stimulating factor and antimicrobial therapy. The purpose of this report is to highlight the importance of immediate appendectomy in neutropenic patients, wherein low absolute neutrophil count should not be considered as a contraindication for urgent surgical procedure.

“…When surgery is required for these patients, possible worsening of the condition and severe postoperative complications should be considered due to neutropenia. Table 1 shows data for abdominal surgeries performed in emergency settings for patients with cyclic neutropenia acquired via searching PubMed using the terms “cyclic neutropenia”, and “operation” [ 1 , 4 6 ]. In these cases, the absolute neutrophil count was as low as 0–200/μL at the first visit.…”

“…Cyclic neutropenia is a disease that causes a neutropenic decrease in peripheral blood in a cycle of about 21 days. It is a rare hereditary disorder with an estimated incidence of 0.5–1 cases per million population and there is no gender difference in the incidence [ 1 ]. Familial cases of autosomal dominant inheritance have been reported, and it may be caused by the neutrophil elastase gene (ELA2) mutation.…”

“…Functioning ileostomy is recommended because of poor healing and risk of infection. Mortality rates range from 2.2 to 48%, and the prognosis depends on the underlying disease and on the clinical conditions of the patient [ 13 , 14 ].…”
