Apoptosis in Lymphocytes of Pancreatic Cancer Patients: Influence of Preoperative Enteral Immunonutrition and Extensive Surgery

Abstract: The mechanisms of correcting immune disorders in patients with pancreatic cancer requiring major surgery procedures by introducing perioperative immune-enhancing diet (immunonutrition) are still unclear. The purpose of our study was to investigate the effect of pancreatic cancer, extensive surgery and immunonutrition versus enteral standard nutrition on the apoptotic signaling pathways. The randomized studies were performed in 72 patients before and after pancreatic cancer resection with preoperative standard … Show more

“…In pancreatectomy, preoperative providing oral immunonutrition helps in reducing risk of postoperative complications such as infections, and also the length of hospital stays . From a molecular viewpoint, the overexpression of caspases and the reduced levels of antiapoptotic proteins in PDAC patients may lead to lymphocyte apoptosis and dysfunction and immune system suppression, especially after pancreatectomy . Therefore, improving perioperative immunonutrition may reduce the risk of immune disorders in patients with PDAC.…”

“…For example, blood lymphocytes are an important factor controlling local immune disorder. The expression of Bcl-2, Bax, NF-κB and PARP-1 in malnourished patients with pancreatic cancer was found to be significantly lower, whereas the expression of caspases and the percentage of cells with death receptors (TNFR1/CD120a and Fas/CD95) were significantly higher [147]. These disorders associated with cancer of the pancreas may lead to a higher lymphocyte susceptibility to accelerated apoptosis that increases immunosuppression.…”
