BackgroundPressure injuries (PIs) are a major problem for healthcare providers, impacting both care costs and patients' quality of life, although they are predominately preventable. These injuries are especially present in Intensive Care Units (ICUs) as a result of the severity of the clinical conditions of patients in this unit.ObjectivesTo develop a simplified version of the Braden scale by removing two of the most subjective subscores—Nutrition and Sensory Perception—in an attempt to reduce the chance of errors by the nursing team during the application of the scale.MethodsA cross‐sectional study was conducted on data collected from patients admitted to the ICU of a private Brazilian tertiary hospital. The resulting data consisted of 5194 patients, 6353 hospital admissions, and 6974 ICU stays. The overall prevalence of PI was 1.09%.ResultsThe T‐test showed that both the Braden and the simplified Braden scores were significantly different between patients with and without PI (p < .001). Patients who developed PIs scored lower than those who did not. The area under the Receiver Operating Characteristic curve of the Braden Scale was 74.21% (95% CI: 68.61%–79.8%) and of the simplified scale was 72.54% (95% CI: 66.87%–78.22%). The Positive Predictive Value of the Braden Scale was 3.17% when interpolated at the same sensitivity as the simplified scale (47.37%), which achieved 3.26%.ConclusionBy removing two of the six subscores of the Braden scale we propose a new tool for identifying patients at risk of developing PI in a more objective and fast way. Our results show that classification performance had little negative impact.Relevance to clinical practiceA simplified, less subjective scale allows for more precise and less time‐consuming risk classification.

Objetivo: evidenciar, através de uma revisão integrativa de literatura, estudos que abordem a presença de lesão por pressão em idosos internados em unidade de terapia intensiva e o papel do enfermeiro na prevenção deste evento. Método: trata-se de uma revisão integrativa da literatura com corte temporal selecionado entre os anos de 2017 a 2022; as bases de dados utilizadas foram LILACS, PUBMED e BDENF. Resultados: após aplicação dos critérios necessários para uma revisão de literatura, a amostra do estudo foi composta por 8 artigos, 37,5% deles foram publicados nos anos de 2017 e 2018 cada, enquanto 25% publicados no ano de 2020, a base de dados mais utilizada foi a PUBMED com 6 artigos; os estudos abordaram aspectos como cuidados e tratamentos na prevenção de LPP em pacientes idosos na UTI, como também os cuidados de enfermagem com o uso de escalas. Conclusão: através desta revisão, foi possível analisar os principais cuidados de enfermagem no que tange a prevenção de lesão por pressão em pacientes idosos internados em UTI, evidenciando a importância do profissional enfermeiro no gerenciamento do cuidado a pacientes críticos.

Introduction: One of the main consequences resulting from the long stay in the scope of Intensive Care Units (ICU) is the occurrence of pressure injuries (PPL), in the vast majority of cases, due to medical devices, such as catheters and probes, as well as such as insufficient blood flow to bony prominences. Objective: In view of the above, the objective was, through the scientific literature, to know the applicability of the Braden scale in the prevention of pressure injuries in the intensive care unit. Methodology: This is an integrative literature review, based on scientific articles related to the topic. The bibliographic search will be carried out from December 2022 to January 2023, in the following databases: LILACS, MEDLINE, IBECS and BDENF, with the following Health Sciences Descriptors (DeCS): “Pressure Injury”; “Risk Measurement”; “Intensive Care Units” using the Boolean operator AND. Inclusion Criteria: complete articles, available for download; published in Portuguese and from the last 05 years. Exclusion criteria: duplicate articles in databases and monographs. The results found were demonstrated in a qualitative way, through information collected through secondary sources of bibliographic review. Results and Discussion: The results showed that the use of the Braden Scale preserves the integrity of the patient's skin, considering that injury prevention is an important component of nursing practice and helps to identify potential risk factors that can lead to to the development of pressure ulcers in ICU patients. In summary, risk assessment instruments and preventive measures such as the Braden Scale can help professionals in the prevention of pressure ulcers. Conclusion: However, prevention of incontinence-related injuries, nutritional diagnosis, nutritional interventions to prevent injuries, and care related to medical devices have been shown to be components of care.
