Aortoiliac Reconstruction with Allograft and Kidney Transplantation as a One-stage Procedure: Long Term Results

Abstract: Our experience suggests that it is possible and safe to use arterial allografts in the treatment of arterial occlusive disease or abdominal aortic aneurysm simultaneously with renal transplantation.

“…Poor results were also observed in another study with 10% perioperative mortality due to myocardial infarction and loss of up to 23% of allografts implanted on a vascular prosthesis [7]. Other studies showed good results of kidney transplantation in association with alloplastic prosthesis replacement procedures [5,6,11,15,16,17,18,19,20,21,22]. Our own experience in 4 cases is in line with these findings.…”

“…To our knowledge, however, the utilization of a fresh allograft to repair an infected aortic aneurysm in the transplant setting has not been described. It is noteworthy that, in contrast to previous studies, the fresh allografts used in both our patient and those reported in Matia's series (17) were all stored in modern preservation solutions (e.g. University of Wisconsin) and transplanted within a mean 12 hours from organ recovery.…”

“…Two small series of simultaneous kidney transplantation and repair of noninfected abdominal aortic aneurysms using fresh arterial allografts (one of which included a mean follow-up interval of 55 months) reported no aortic allograftrelated morbidity or mortality (16,17). Moreover, the utilization of donor thoracoabdominal aortic grafts as conduits for multivisceral and pediatric liver transplantation is well-established (18).…”

“…15,16 Their good resistance to infection was documented even in one of our patients after simultaneous AAA repair and renal transplantation in whom an ischemic colitis with colon perforation developed during early postoperative period. 17 To our knowledge, this report describes the first case of a patient awaiting renal transplantation with concomitant multilevel AAs who was successfully treated for all three pathologies. His treatment required the close cooperation of anesthesiologists as well as cardiovascular, vascular, and transplant surgeons to achieve this outcome.…”
