Aortic valve graft implantation in rats: a new functional model

Abstract: Compensated native aortic insufficiency results in aortic diastolic flow reversal distal to the infrarenal aorta, thus allowing normal motion of the infrarenal allograft leaflets. This functional model will provide an opportunity to investigate the role of immunologic valve injury in the failure of valve allografts.

“…Of note, these techniques are limited by placing donor valve in series with the recipients' native aortic valve. 21 This results in abnormal blood flow patterns and removes the pressure gradient necessary to open and close the donor valve. 22 In order to avoid size mismatch of the anastomoses during implantation, Lee et al developed a rodent model where a donor pulmonary valve is transplanted orthotopically into a second donor heart.…”

“…Therefore, Yankah's technique was refined by Green et al, 19 who used the innominate artery instead of the ascending aorta to create the distal anastomosis, and Zhou et al, 20 anastomosed the donor aortic arch to the aortic annulus, so the subclavian artery can be used for the proximal anastomosis. Of note, these techniques are limited by placing donor valve in series with the recipients' native aortic valve 21 . This results in abnormal blood flow patterns and removes the pressure gradient necessary to open and close the donor valve 22 .…”

“…1 To overcome this difficulty Légaré and Ross suggest the use of a functional rat implantation model in which the creation of native aortic valve insufficiency results in a significant diastolic flow reversal in the abdominal aorta, allowing leaflets to move and function. 2 Although we were aware of the existence of this model, we decided not to use it for our study. One reason is that the creation of aortic insufficiency results in progressive congestive heart failure from volume overload, making the model unsuitable for long-term follow-up studies.…”

“…In a previous article, they reported performing coronary artery bypass grafting first, then MVR, and finally LVR. 2 Ischemic mitral regurgitation is a functional disorder that is due mainly to changes in the geometric relationships between structurally normal valve components and a severely damaged ventricle. By changing ventricular volume and geometry, LVR may unpredictably result in mitral insufficiency that is absent before surgery 3 or in an amelioration (sometimes disappearance) of mitral regurgitation.…”
