Aortic Dissection: A Review of the Pathophysiology, Management and Prospective Advances

Abstract: : Aortic dissection is an emergent medical condition, generally affecting the elderly, characterized by a separation of the aortic wall layers and subsequent creation of a pseudolumen that may compress the true aortic lumen. Predisposing factors mediate their risk by either increasing tension on the wall or by causing structural degeneration. They include hypertension, atherosclerosis, and a number of connective tissue diseases. If it goes undetected, aortic dissection carries a significant mortality risk; the… Show more

“…Aortic coarctation is a state in which blood in the aortic lumen enters the aortic mesentery from a tear in the aortic intima, causing the mesentery to split and expand in the direction of the long axis of the aorta forming a true/false separation of the two lumens of the aortic wall [ 10 ]. The peak age of onset of aortic coarctation is 50–70 years, with a male to female ratio of about 2–3:1.…”

“…Aortic dissection (AD) is a life-threatening cardiovascular disease with a high mortality rate. 1,2 Aortic dissection can be classified into Stanford type A or type B based on the involvement of ascending aorta or not. 3 Notably, AD patients present a higher incidence of men than females (around 1.7:1 male to female ratio).…”

“…This effect may be important for the formation of atherosclerotic lesions ( 34 ). Recent studies on human vascular lesions have demonstrated that apoptosis is a prominent feature of both AS and restenosis, which are one of the causes leading to AD ( 35 , 36 ). Therefore, AD may be further aggravated after CTGF induces SMC apoptosis.…”
