ÖZETAmaç: Aort balon valvüloplasti (ABV) yapılan aort kapak darlığı (AKD) olan olgularda orta dönem sonuçlarının değerlendirilmesi ve aort yetersizliği (AY) gelişimini etkileyen faktörlerin belirlenmesi amaçlandı. Yöntemler: Eylül 2003-Ocak 2010 tarihleri arasında 97 olguya yapılan toplam 114 ABV girişimi retrospektif olarak irdelendi. Hastalar işlem önce-si ve sonrasında transvalvüler gradiyentleri, işlem sonrası yeni aort yetersizliği gelişimi veya mevcut yetersizliğin artışı ve gerek işlemin başarı-sını gerekse aort yetersizliği gelişimini etkileyen unsurlar yönünden ele alındı. Ayrıca takipte yeniden daralma, aort yetersizliklerinde artış veya azalış bulguları değerlendirildi. İstatistiksel analiz için Student t-testi, tekrarlayan ölçümler için varyans, doğrusal ayırıcı ve Kaplan-Meier sağ-kalım analizleri kullanıldı. Bulgular: Olguların yaş ortalaması 6.63±6.33 yıl (2 gün-21 yıl), izlem süresi 32.6±25.1 (1-75) ay idi. Olguların 18'i yenidoğan döneminde olmak üzere, 37'si bir yaşından küçüktü. ABV öncesi 77.2±24.9 mmHg olan pik sistolik gradiyent 28.5±12.9 mmHg'ya düştü (p=0.0001). Seksen iki girişim optimal, 29'u suboptimal, 3 girişim başarısız olarak sonuçlandı. On altı girişimde belirgin düzeyde olmak üzere, toplam 53 girişimde AY'de artış oldu. Balon çapı/anülüs çapı oranı ve gradiyentin azalma yüzdesi belirgin AY saptanan girişimlerde diğerlerine göre anlamlı olarak yüksekti (sırasıyla p=0.02 ve p=0.03). Ayrıca, süt çocukluğu döneminde (<1 yaş) ABV yapılanlarda (n=37) belirgin AY artışı sıklığı (9/37, %24), 1 yaş üstü (n=77) girişimlere (7/77, %9) göre anlamlı oranda yüksek bulundu (p=0.02). İşlem sonrası 3'ü yenidoğan, 4 hasta yoğun bakım ünitesin-de kaybedildi. Sonuç: Aort balon valvüloplasti, AKD'nın tedavisinde etkin ve güvenilir bir palyasyondur. En sık rastlanan komplikasyon olan AY yüksek balon çapı/anülüs çapı oranı ve girişim sonrası gradiyentin azalma yüzdesi ile ilişkili bulunmuş, ayrıca bir yaş altı olgularda daha sık gözlenmiştir.
ABSTRACTObjective: The objective of this study was to assess medium-term results of aortic balloon valvuloplasty (ABV) in congenital aortic stenosis and to determine factors affecting development of aortic regurgitation (AR). Methods: Between September 2003 and January 2010, 114 ABV procedures performed on 97 patients analyzed retrospectively. Patients were evaluated in terms of transvalvular gradients before and after procedure, development of new aortic insufficiency or increase in the current insufficiency and factors affecting the procedural success and development of aortic insufficiency. In addition, follow up results were also evaluated in terms of restenosis and increase or decrease of aortic regurgitation. Student`s t-test, ANOVA for repeated measurements, linear discriminant and Kaplan-Meier survival analyses were used for statistical analysis. Results: The mean age was 6.63±6.33 year (2 days-21 years). Mean follow-up was 32.6±25.1 months (1 to 75 months). Thirty-seven patients were infant (<1 year) and 18 of them were newborn.
