Objective Exposure to endotoxin has been consistently associated with a reduced risk of lung cancer. However, there is a paucity of information regarding temporal aspects of this relationship. The objective of this study was to investigate the associations between contiguous windows of endotoxin exposure and risk of lung cancer. Methods Data were reanalyzed from a case-cohort study (602 cases, 3,038 subcohort) of female textile workers in Shanghai, China. Cumulative endotoxin exposure was partitioned into two windows: ≥20 and <20 years before risk. Exposure–response relations were examined using categorical and non-linear (semi-parametric) models, accounting for confounding by previous exposure windows. Results There was an inverse trend of decreasing risk of lung cancer associated with increasing levels of endotoxin exposure ≥20 years before risk (p trend = 0.02). Women in the highest two categories of cumulative exposures had hazard ratios of 0.78 (95% CI 0.60–1.03) and 0.77 (95% CI 0.58–1.02) for lung cancer, respectively, in comparison with unexposed textile workers. There was, however, a weaker association and not statistically significant between lung cancer and endotoxin exposure accumulated in the more recent window (<20 years before risk). Conclusion Results provide further evidence that endotoxin exposure that occurred 20 years or more before risk confers the strongest protection against lung cancer, indicating a possible early anti-carcinogenic effect. Further studies are needed to better understand the underlying biological mechanisms for this effect.

Dysregulated immune function is involved in the pathogenesis of many common human diseases. Living in urban, microbe-poor environment may have a profound influence on the immune function and eventually also on carcinogenesis. Unfortunately, few studies have thus far addressed the role of exposure to the environmental microbiota on the risk of cancer. Which mechanisms are broken in individuals prone to develop chronic inflammation in response to exposure that does not cause harm in others? Recent work in immunology has revealed that Th17 cells, a third subset of Th cells, and inflammatory cytokines, particularly IL-23, are closely linked with tumour-associated inflammation. Albeit the precise role of Th17 cells in cancer is still unclear and a matter of debate, accumulating evidence shows that Th17 cells are enriched in a wide range of human tumours, and that these tumour-derived Th17 cells may promote angiogenesis, tumour growth and inflammation. Regulatory T cells, in turn, appear to have counter-regulatory effects on Th17 cells and can inhibit their function. Thus, the regulatory network, induced and strengthened by continuous exposure to environmental microbiota, may play an important role in tumour immunobiology in preventing the establishment of chronic inflammation in its early phases. In addition, the discovery of the Toll-like receptor (TLR) system has brought micro-organisms to new light; continuous signalling via these TLRs and other receptors that sense microbial components is necessary for epithelial cell integrity, tissue repair, and recovery from injury. In this communication, we summarise the epidemiological data of living in environments with diverse microbial exposures and the risk of cancer, and discuss the related immunological mechanisms, focusing on the links between environmental microbiota, the Th17/IL-23 axis and cancer-associated inflammation.

Daugiamečiai natūralaus imuniteto endotoksinui tyrimai, pradedant nuo 1970 m., atlikti Vilniaus universiteto Onkologijos instituto Profilaktinės imunologijos laboratorijoje, parodė, kad natūralaus imuniteto funkcijos tiek imunostimuliuotų, tiek imunosupresuotų organizmų yra glaudžiai susijusios su specifiniais IgM klasės natūraliais antikūnais (IgMNA) enterobakteriniam bendram antigenui (EBA), esančiam žarninės grupės mikroorganizmų išorinėje membranoje, t. y. endogeniniai ar spontaniniai antikūnai gram-neigiamos bakterijos Alcaligenes faecalis 415 lipopolysacharido molekulėms, vadinamoms endotoksinu (IgMNAE). Šie duomenys, remiantis mūsų anksčiau sukurtos vėžio kilmės rezistentiškumo teorijos biologiniais dėsniais (rezistentiškumas ir atavizmas), leido pasiūlyti bendrą hipotezę apie "snaudžiančio" atavistinio endotoksino galimą aktyvaciją ir svarų vaidmenį vėžio patologijoje navikinėms ląstelėms tampant savarankiška parazitine populiacija. Hipotezės išvados buvo ekstrapoliuotos į žmogaus vėžio profilaktiką. 1979 ir 1982 m. tirtų 2 223 sveikų žmonių (kraujo donorų ir ne donorų) kraujo serumo IgMNAE duomenys, nustatyti precipitacijos reakcijos agaro gelyje ir autoimunografijos metodais, buvo įvertinti vėžio ligotumo atžvilgiu. Turėję IgMNAE žmonės po 15-30 metų susirgo onkologinėmis ligomis atitinkamai procentine išraiška 1/3 mažiau (5 proc.) negu jų neturėję (p < 0,05). Gauti duomenys leidžia apibendrinti, kad natūralaus imuniteto endotoksinui mechanizme galima įžvelgti organizmo homeostazės apsauginę reakciją. IgMNAE aukštas lygis pagal paveldo programą turėtų stabdyti kancerogenais pažeistų ląstelių evoliucijos eigoje "paslėpto" ląstelės gelmėse ir kancerogenezės procese "atgyjančio" parazitizmo nešėjo atavistinio endotoksino veikos suaktyvėjimą, būtiną vėžinių ląstelių išgyvenimui ir metastazavimui, bet antagonistišką sveikoms kūno ląstelėms. Todėl natūralaus imuniteto endotoksinui stimuliacija pagamintomis vakcinomis prieš atavistinį endotoksiną gali būti naudinga vėžio prevencijoje žmonėms su žemu natūralaus imuniteto lygmeniu ir taip pat kuriant imunoterapinius metodus, siekiant sustabdyti navikinių ląstelių endogeninį parazitizmą. abstract Key words: cancer, endotoxin, immunity, prophylaxis, conception.
