Antipsychotic prescribing patterns on admission to and at discharge from a tertiary care program for treatment-resistant psychosis

Abstract: Retrospective data were collected from 330 individuals who were treated at a tertiary care program for treatment-resistant psychosis between 1994 and 2010. The main objectives were to compare the use of antipsychotic monotherapy to polypharmacy and to characterize within-individual changes in treatment and symptomatology between admission and discharge. At admission, individuals who were prescribed only one antipsychotic were comparable to those who were prescribed at least two antipsychotics with regard to de… Show more

“…However, currently available evidence suggests that APP is strongly associated with high/excessive dosing trends in routine practice. 35 According to the previously mentioned large 10-year cohort study, 16 the daily doses of APs were substantially increased in both calculations "DDD" and "CPZeq" during the 10 year periods. The yearly DDD in AMP patients also significantly increased, however, it was found to double in APP patients indicating that APP is mainly utilized for augmenting inadequate efficacy rather than counteracting SEs.…”

“…12 Thus, hospitalisations could be considered appropriate moments for deprescribing, because healthcare professionals can closely monitor the patients, and successful hospital-based deprescribing programmes for antipsychotics have been described. 26,27 Prior use of clozapine and having already tried at least two different antipsychotics increased the risk of being prescribed APP. These findings suggest that TRS or subjective TRS (i.e.…”

“…The SGA clozapine is the preferred drug for treatment resistant psychosis (11)(12)(13)(14)(15), producing therapeutic responses in approximately 30% of patients previously refractory to other antipsychotics (16)(17)(18), possibly reflecting a unique mechanism of action based on its complex pharmacology (19).…”
