Antiproliferative Effect of Somatostatin Analogs – Data Analyses and Clinical Applications in the Context of the CLARINET Study

Abstract: Klinika komplexní onkologické péče, Masarykův onkologický ústav, Brno RECAMO, Masarykův onkologický ústav, Brno SouhrnVýchodiska: Somatostatinová analoga (SSA) jsou již více než dvě desetiletí využívána k léčbě hormonálních syndromů spojených s neuroendokrinními nádory. V posledních letech dvě randomizované placebem kontrolované klinické studie fáze III prokazují jejich antiproliferační účinek. Protinádorový účinek oktreotidu LAR (long-acting repeatable) byl prokázán v randomizované, placebem kontrolované stud… Show more

“…For early stage PHNEC, surgical resection of liver tumor tissue or partial hepatectomy is the most common treatment [ 7 ], with a five-year survival of 74–78% [ 4 ]. Otherwise, treatment options are limited to liver transplantation, TACE, or administration of somatostatin [ 11 , 38 – 42 ]. Neither of our two cases was eligible for surgical resection of the tumor, and liver transplantation was not performed due to a lack of matched donor livers or financial concerns.…”

“…By interrupting the blood supply to the tumor while directly delivering chemotherapeutics, TACE inhibits tumor growth and extends survival. Somatostatin analogues, such as octreotide, can inhibit secretion of growth hormone by NECs and prevent tumor proliferation [ 11 ].…”

“…Those agents demonstrate antiproliferative action and reduce glucagon secretion. In the recent study lanreotide was proven to significantly prolong progression-free survival and is recommended for the treatment of patients with gastroenteropancreatic neuroendocrine tumours (grade 1 and 2) (Ki-67 proliferative index < 10%), regardless of the degree of liver involvement and the location of primary tumour [ 16 ]. Our patient complied with all of these criteria and lanreotide was used with good initial response and tolerability, which resulted in almost complete resolution of skin lesions.…”
