SUMMARYSmall colony variants (SCVs) were first described more than 100 years ago forStaphylococcus aureusand various coagulase-negative staphylococci. Two decades ago, an association between chronic staphylococcal infections and the presence of SCVs was observed. Since then, many clinical studies and observations have been published which tie recurrent, persistent staphylococcal infections, including device-associated infections, bone and tissue infections, and airway infections of cystic fibrosis patients, to this special phenotype. By their intracellular lifestyle, SCVs exhibit so-called phenotypic (or functional) resistance beyond the classical resistance mechanisms, and they can often be retrieved from therapy-refractory courses of infection. In this review, the various clinical infections where SCVs can be expected and isolated, diagnostic procedures for optimized species confirmation, and the pathogenesis of SCVs, including defined underlying molecular mechanisms and the phenotype switch phenomenon, are presented. Moreover, relevant animal models and suggested treatment regimens, as well as the requirements for future research areas, are highlighted.

Staphylococcus aureus is a leading cause of both nosocomial and community-acquired infection. Biofilm formation at the site of infection reduces antimicrobial susceptibility and can lead to chronic infection. During biofilm formation, a subset of cells liberate cytoplasmic proteins and DNA, which are repurposed to form the extracellular matrix that binds the remaining cells together in large clusters. Using a strain that forms robust biofilms in vitro during growth under glucose supplementation, we carried out a genome-wide screen for genes involved in the release of extracellular DNA (eDNA). A high-density transposon insertion library was grown under biofilm-inducing conditions, and the relative frequency of insertions was compared between genomic DNA (gDNA) collected from cells in the biofilm and eDNA from the matrix. Transposon insertions into genes encoding functions necessary for eDNA release were identified by reduced representation in the eDNA. On direct testing, mutants of some of these genes exhibited markedly reduced levels of eDNA and a concomitant reduction in cell clustering. Among the genes with robust mutant phenotypes were gdpP, which encodes a phosphodiesterase that degrades the second messenger cyclic-di-AMP, and xdrA, the gene for a transcription factor that, as revealed by RNA-sequencing analysis, influences the expression of multiple genes, including many involved in cell wall homeostasis. Finally, we report that growth in biofilm-inducing medium lowers cyclicdi-AMP levels and does so in a manner that depends on the gdpP phosphodiesterase gene.Staphylococcus aureus | biofilm | eDNA | cyclic-di-AMP

В настоящее время все большую акту-альность приобретает проблема оппортуни-стических инфекций, возбудителями которых являются условно-патогенные (оппортуни-стические, потенциально патогенные) микро-организмы (УПМ). Это большая группа раз-нородных по систематическому положению микробов, которые вступают с организмом че-ловека в одних случаях в отношения симбио-за, комменсализма и/или нейтрализма, в дру-гих -в конкурентные отношения, нередко приводящие к развитию заболевания. В совре-менной патологии человека большее значение имеют представители родов Escherichia, Proteus, Klebsiella, Enterobacter, Serratia, Staphylococcus, Streptococcus, Peptococcus, Peptostreptococcus, Haemophilus, Pseudomonas, Acinetobacter, Moraxella, Bacteroides, Candida, Aspergillus и др. [1,2].Важность исследования УПМ при рев-матических заболеваниях (РЗ) может быть связана с изменением этиологической струк-туры инфекций в целом. Антропогенное воз-действие, будучи мощным фактором эволю-ции живой материи, привело в последние го-ды к стиранию грани между УПМ и облигат-но-патогенными микроорганизмами, возбу-дителями «классических» инфекций [2]. УПМ обладают определенной вирулентностью -сложным и часто многофакторным феноме-ном, детерминированным большим количе-ством локусов, способных играть ведущую роль при многих патологических состояниях [2-4]. Так, геномный и молекулярный анализ О б з о р ы Представлены данные литературы о роли оппортунистических микроорганизмов при ревматических заболе-ваниях (РЗ). Предполагается возможное участие условно-патогенных микроорганизмов (УПМ) в качестве триггерных факторов, инициирующих развитие хронического воспаления. Наряду с этим УПМ при аутоим-мунных заболеваниях могут играть защитную роль за счет взаимодействия с Toll-like рецепторами и актива-ции T-клеток, обладающих супрессорной активностью. Возможность участия УПМ в патогенезе РЗ под-тверждает не только выделение микроорганизмов, но и обнаружение антибактериальных антител различных классов. Велико значение УПМ в этиологии коморбидных инфекций, риск развития которых обусловлен как наличием аутоиммунного РЗ, так и необходимостью применения препаратов, обладающих иммуносу-прессивным действием. Активное внедрение в клиническую практику генно-инженерных биологических препаратов сопровождается нарастанием частоты и тяжести течения различных инфекций, в том числе вы-званных УПМ. Оппортунистические микроорганизмы, обладая выраженной биологической и экологиче-ской пластичностью, способны длительно персистировать в условиях изменения иммунной защиты орга-низма больных РЗ. Приводятся данные о повышении адгезивных свойств и персистентного потенциала ми-кроорганизмов, колонизирующих организм больных РЗ. При РЗ выделяются УПМ, которые отличаются вы-раженной полирезистентностью к антибиотикам, что затрудняет лечение и профилактику оппортунистиче-ских инфекций в ревматологии.
