Antimicrobial Stewardship Program in a Pediatric Intensive Care Unit

Abstract: We conducted an antimicrobial stewardship program in a pediatric intensive care unit. An interrupted time-series analysis revealed a significant reduction in level and trend of days of therapy per 1000 patient-days for antipseudomonal agents. No increase in adverse patient outcomes was found.

“…11 Hesitation by physicians to de-escalate or stop antimicrobials because of disease severity, especially in highrisk patients, can be challenging for ASP teams. 23 Thus, demonstrating that stewardship efforts, including the recommendation to stop therapy, do not result in increased morbidity or mortality is important. A recent systematic review and meta-analysis of audit with feedback in the ICU setting, using mortality as the outcome, did not identify an increased risk of death with ASP prospective audit with feedback.…”

“…Prospective audit and feedback has previously been proposed as an antibiotic stewardship intervention in adults and children [24][25][26] . However, the impact of this type of intervention in the PICU setting is not known, with only a limited number of published reports related to PICUs 3,[17][18][19][20][21] . In this study, complete or partial adherence (at least one out of two recommendations followed) to the given antibiotic stewardship recommendations was 79%.…”

“…5,7 After 5 years of handshake stewardship, MacBrayne et al 8 reported 25% reduction in antimicrobial use, demonstrating sustainable efficacy of handshake stewardship. Variations in handshake stewardship exist in numerous centers, targeting different services, [9][10][11] or entire hospitals. 12,13 Anecdotally, the success of handshake stewardship lies in the ongoing face-to-face interaction between the antimicrobial stewardship program and medical teams, fostering collaboration and promoting real-time education about optimal antimicrobial management.…”
