Antimicrobial resistance trends in bacterial keratitis over 5 years in Sydney, Australia

Abstract: Importance Antimicrobial resistance (AMR) patterns in bacterial keratitis may fluctuate in a geographic location over time. Background To investigate any change in AMR patterns in Sydney, Australia. Design Retrospective case series. Participants All patients with microbial keratitis who underwent a corneal scrape and culture from 2012 to 2016 at the Sydney Eye Hospital. Methods Matrix‐assisted laser desorption ionization‐time of flight (MALDI‐TOF) mass spectrometry identified organisms. The Calibrated Dichotom… Show more

“…Gentamicin is commonly prescribed in S. aureus ocular infections, but its susceptibility rates vary [41]. The current results are consistent with other studies from the USA and Australia for S. aureus ocular isolates [10,[42][43][44]. The antibiotic susceptibility profile in the current study suggests gentamicin to be the best option to treat S. aureus ocular infections in both Australia and the USA, and vancomycin to be reserved to treat isolates that are resistant to other antibiotics.…”

“…3 , 5 , 6 To make matters worse, the increasing emergence of multi-drug-resistant pathogens is another major challenge leading to higher rates of morbidity. 22 24 In severe cases of medically uncontrollable infectious keratitis, surgical interventions and corneal keratoplasty are the last therapeutic alternatives; however, these are often associated with poor visual outcomes and increased risks of graft rejection/failure. 25 27 Moreover, major shortage of corneal graft, with only one in 70 patients worldwide having access to donor tissue is an additional challenge.…”

“…Only one of the UK studies mentioned above reported MRSA incidence; two cases were found over 10 years [8]. Two studies from Sydney, Australia, report similar findings to the UK studies, with coagulase-negative staphylococci being the most commonly discovered bacteria and low rates of MRSA within S. aureus isolates [17,18]. Meanwhile, studies from Mexico, Taiwan and South India confirm a statistically significant increase in MRSA incidence over time [19][20][21].…”
